Overview of the medicine
Vorifast 200 mg Tablet is an antifungal medication used to treat serious fungal infections caused by susceptible strains of fungi.
Uses & Indications
Dosage
Adults
Loading dose: 400 mg every 12 hours for 2 doses. Maintenance dose: 200 mg every 12 hours, adjusted based on patient response and tolerability.
Elderly
No dose adjustment is generally required, but caution is advised due to potential age-related organ function decline.
Renal_impairment
No dose adjustment is required for patients with mild to severe renal impairment (CrCl ≥ 30 mL/min). Monitor carefully. For severe impairment, consider alternative therapies.
How to Take
Take on an empty stomach, at least one hour before or one hour after a meal. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Voriconazole inhibits fungal cytochrome P450-mediated 14 alpha-lanosterol demethylation, an essential step in fungal ergosterol biosynthesis. This leads to the accumulation of 14 alpha-methyl sterols and a decrease in ergosterol, causing disruption of fungal membrane function and growth.
Pharmacokinetics
Onset
Typically within 1-2 hours for therapeutic levels.
Excretion
Primarily via hepatic metabolism, with less than 2% excreted unchanged in urine.
Half life
Variable, dose-dependent, but generally 6 hours to 10 hours at therapeutic doses.
Absorption
Well absorbed orally, bioavailability approximately 90%.
Metabolism
Extensively metabolized by hepatic cytochrome P450 enzymes (CYP2C19, CYP2F9, CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to voriconazole or any component of the formulation
- Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, rifampin, carbamazepine, phenobarbital, sirolimus, or ergot alkaloids

Rifampin
Significant decrease in voriconazole plasma concentrations.
Phenytoin
Significant decrease in voriconazole plasma concentrations, monitor phenytoin levels.
Cyclosporine
Increased cyclosporine levels, monitor cyclosporine levels and adjust dose.
Carbamazepine
Significant decrease in voriconazole plasma concentrations.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, discontinue voriconazole and provide supportive care. Voriconazole is hemodialyzable.
Pregnancy & Lactation
Pregnancy Category D. Should not be used during pregnancy unless the benefits outweigh the potential risks. Not recommended during breastfeeding as it may be excreted in breast milk.
