Description
Overview of the medicine
Vtapa 0.1% Cream contains tacrolimus, an immunosuppressant, and is used to treat moderate to severe atopic dermatitis (eczema) in adults and adolescents (16 years and older) who are not adequately responsive to or are intolerant of conventional therapies.
Uses & Indications
Dosage
Adults
Apply a thin layer of Vtapa 0.1% Cream to the affected skin areas twice daily. Continue treatment for 1-3 weeks or until clearance of lesions, then reduce frequency as directed by a physician for maintenance.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required due to minimal systemic absorption.
How to Take
For topical dermatological use only. Apply a thin layer to the affected skin areas and rub in gently and completely. Do not apply on broken skin or mucous membranes. Wash hands thoroughly after application.
Mechanism of Action
Tacrolimus inhibits calcineurin, a phosphatase involved in T-cell activation. By blocking calcineurin, it prevents the transcription of various pro-inflammatory cytokines, thereby reducing inflammation and immune response in the skin.
Pharmacokinetics
Onset
Improvement in symptoms can be seen within 1-2 weeks of treatment.
Excretion
Mainly through feces if systemically absorbed, but negligible for topical use.
Half life
Not well-defined for topical application due to minimal systemic absorption.
Absorption
Minimal systemic absorption after topical application; primarily acts locally on the skin. Systemic exposure is generally very low.
Metabolism
Primarily hepatic (CYP3A4) if systemically absorbed, but systemic absorption from topical application is negligible.
Side Effects
Contraindications
- •Hypersensitivity to tacrolimus, macrolides, or any component of the cream.
- •Patients with Netherton's syndrome or other widespread skin conditions where systemic absorption might be increased.
- •Application to areas with active viral skin infections (e.g., herpes simplex, varicella).
Drug Interactions
Live vaccines
Avoid live vaccines during treatment, especially in patients with extensive lesions, due to theoretical risk of reduced immune response.
Systemic Immunosuppressants
Concurrent use with systemic immunosuppressants is not recommended due to potential for additive immunosuppression.
Storage
Store below 30°C. Protect from light and moisture. Keep out of the reach of children.
Overdose
Overdose is unlikely with topical application due to minimal systemic absorption. In case of accidental ingestion, seek medical attention.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether tacrolimus is excreted in human milk after topical application; exercise caution when administering to a nursing mother.
