Vulcan 150 mg Capsule contains Fluconazole, an antifungal medication used to treat a variety of fungal and yeast infections. It works by stopping the growth of fungi. It is commonly prescribed for vaginal yeast infections, oral thrush, esophageal candidiasis, and certain systemic fungal infections.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: A single oral dose of 150 mg. For other indications, dosage varies (e.g., 50-400 mg daily) and duration depends on the infection type and severity.
Elderly
No dosage adjustment needed unless there is renal impairment. Dose should be adjusted based on creatinine clearance.
Renal_impairment
Dosage should be reduced based on creatinine clearance. For CrCl < 50 mL/min, dosage should be halved after the initial loading dose.
How to Take
Take the capsule orally with or without food. Swallow whole with a glass of water. For a single dose for vaginal candidiasis, take it at any time of the day.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14α-demethylation of lanosterol, an essential step in fungal ergosterol biosynthesis. This inhibition leads to a disruption of fungal cell membrane integrity, thereby inhibiting fungal growth and replication.

Excretion
Primarily renal; about 80% of the administered dose is excreted unchanged in the urine.

Approximately 30 hours, allowing for once-daily dosing for many indications.

Side Effects
Contraindications
- •Hypersensitivity to fluconazole or any of its excipients, or to other azole antifungals.
- •Co-administration with terfenadine, cisapride, astemizole, pimozide, or quinidine due to risk of QTc prolongation and potential for serious cardiac events.
Drug Interactions
Warfarin
May increase prothrombin time (INR); monitor closely.
Phenytoin
Increases phenytoin levels; monitor phenytoin levels and adjust dosage.
Rifampicin
Decreases fluconazole levels; consider increasing fluconazole dose.
Cyclosporine
Increases cyclosporine levels; monitor cyclosporine levels.
Oral Sulfonylureas
May prolong half-life; monitor blood glucose.
Hydrochlorothiazide
Increases fluconazole levels; no adjustment usually needed.
Benzodiazepines (e.g., Midazolam, Triazolam)
Increases levels; dose reduction may be necessary.
Storage

Overdose
In case of overdose, supportive treatment should be initiated. Gastric lavage may be considered. Hemodialysis may enhance the elimination of fluconazole.
