Description
Overview of the medicine
Wella 125 mg Tablet contains Azithromycin, a macrolide antibiotic used to treat a variety of bacterial infections, including respiratory tract infections, skin infections, and certain sexually transmitted infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Depending on the infection, typically 500 mg once daily for 3 days, or 500 mg as a single dose on day 1 followed by 250 mg once daily for 4 days. For 125 mg, it's often part of a pediatric regimen or specific adult indications at lower doses.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal and hepatic function. However, caution should be exercised due to potential for QT prolongation.
Renal_impairment
No dosage adjustment is needed for mild to moderate renal impairment (creatinine clearance > 10 mL/min). Data on severe impairment is limited; use with caution.
How to Take
Take the tablet orally, with or without food. It is generally recommended to take it at the same time each day. Do not crush or chew the tablet.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting bacterial protein synthesis and ultimately stopping bacterial growth. It is primarily bacteriostatic but can be bactericidal at high concentrations against sensitive strains.
Pharmacokinetics
Onset
Peak plasma concentrations reached in 2-3 hours.
Excretion
Primarily excreted unchanged in the bile, with a small amount excreted via urine (approximately 6% of an oral dose).
Half life
Terminal elimination half-life is 2-4 days due to extensive tissue uptake and slow release.
Absorption
Rapidly absorbed from the gastrointestinal tract, though bioavailability is approximately 37% due to extensive tissue distribution.
Metabolism
Partially metabolized in the liver by N-demethylation; however, a large portion is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum digoxin levels.
Warfarin
Increased anticoagulant effect; monitor prothrombin time (INR).
Nelfinavir
Increases azithromycin serum concentrations.
Antacids containing aluminum or magnesium
Reduce peak plasma levels; take azithromycin at least 2 hours before or 4 hours after antacids.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and general supportive measures are indicated.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human milk; caution should be exercised when administered to a nursing woman.
