Overview of the medicine
Winolip 180 mg Tablet is a lipid-lowering medication used to reduce high cholesterol and triglyceride levels in the blood. It belongs to a group of medicines called fibrates. It works by reducing the production of 'bad' cholesterol (LDL) and triglycerides and increasing the production of 'good' cholesterol (HDL).
Uses & Indications
Dosage
Adults
180 mg once daily with food. Dosage should be individualized based on patient response and lipid levels.
Elderly
Similar to adult dosage, but caution should be exercised and renal function monitored.
Renal_impairment
Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73m²). For moderate impairment (eGFR 30-59 mL/min/1.73m²), consider a lower dose or avoid if possible.
How to Take
Take orally, once daily with a meal to enhance absorption. Swallow the tablet whole; do not crush or chew.
Mechanism of Action
Fenofibrate, a fibric acid derivative, acts by activating peroxisome proliferator-activated receptor alpha (PPAR-alpha). This activation leads to increased lipolysis and elimination of triglyceride-rich particles from plasma by activating lipoprotein lipase and reducing the production of apoprotein C-III. It also increases the synthesis of apoproteins A-I and A-II, leading to an increase in HDL-cholesterol.
Pharmacokinetics
Onset
Lipid-lowering effects typically observed within 2-5 days, maximal effects by 4-8 weeks.
Excretion
Mainly excreted in the urine as fenofibric acid and its glucuronide conjugate. A small amount is excreted in the feces.
Half life
Approximately 20-23 hours for the active metabolite (fenofibric acid).
Absorption
Well absorbed after oral administration, especially with food. Peak plasma levels reached within 4-5 hours.
Metabolism
Fenofibrate is rapidly hydrolyzed by esterases to its active metabolite, fenofibric acid, which is then conjugated with glucuronic acid.
Side Effects
Contraindications
- Severe renal impairment (eGFR < 30 mL/min/1.73m²)
- Active liver disease, including primary biliary cirrhosis and unexplained persistent liver function abnormalities
- Pre-existing gallbladder disease
- Hypersensitivity to fenofibrate or any component of the formulation
- Known photoallergy or phototoxic reaction during treatment with fibrates or ketoprofen
Drug Interactions
Warfarin
Potentiates the anticoagulant effect of warfarin, increasing the risk of bleeding. Monitor INR closely.
Cyclosporine
Increased risk of renal dysfunction. Monitor renal function.
Statins (HMG-CoA Reductase Inhibitors)
Increased risk of myopathy/rhabdomyolysis. Use with caution, especially at higher doses of statins.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Treatment of overdose should be symptomatic and supportive. Gastric lavage or activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding due to lack of data on excretion in human milk.
