Description
Overview of the medicine
Xanbac 10 mg tablet contains Xanbacine, a GABA-A receptor modulator, primarily used for the short-term management of anxiety disorders, insomnia, and as a pre-operative sedative. It helps to calm the brain and nerves.
Uses & Indications
Dosage
Adults
Generalized anxiety disorder: Initially 5 mg, 2-3 times daily, adjusted to a usual dose of 10-20 mg daily in divided doses. Panic disorder: Initially 5 mg once daily at bedtime, adjusted up to 30 mg daily.
Elderly
Initial dose should be half of the adult dose (e.g., 2.5 mg 2-3 times daily), adjusted carefully due to increased sensitivity and risk of adverse effects.
Renal_impairment
Use with caution. Dosage reduction may be necessary in severe impairment. Start with lower doses.
How to Take
Take orally with or without food. Do not crush or chew the tablet. Swallow whole with water.
Mechanism of Action
Xanbacine selectively binds to GABA-A receptors in the central nervous system, enhancing the inhibitory effects of gamma-aminobutyric acid (GABA). This leads to increased chloride ion influx into neurons, resulting in hyperpolarization and reduced neuronal excitability, thus producing anxiolytic, sedative, hypnotic, and muscle relaxant effects.
Pharmacokinetics
Onset
30-60 minutes
Excretion
Primarily excreted in the urine as metabolites.
Half life
Approximately 10-14 hours (active metabolites may extend duration)
Absorption
Rapidly and completely absorbed after oral administration, peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver via CYP3A4 enzymes, forming active and inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to Xanbacine or other benzodiazepines
- Acute narrow-angle glaucoma
- Severe respiratory insufficiency
- Severe hepatic impairment
- Myasthenia gravis
Drug Interactions
Opioids
Increased risk of respiratory depression, profound sedation, coma, and death.
Antifungals (e.g., Ketoconazole)
May inhibit metabolism of Xanbacine, leading to increased plasma levels and enhanced effects.
Alcohol and other CNS depressants
Potentiation of sedative effects, increased risk of respiratory depression.
CYP3A4 inducers (e.g., Rifampicin)
Can decrease Xanbacine levels and efficacy.
CYP3A4 inhibitors (e.g., Grapefruit juice)
Can increase Xanbacine levels and effects.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms include somnolence, confusion, impaired coordination, diminished reflexes, and coma. Management involves supportive care, maintaining airway, and monitoring vital signs. Flumazenil may be used as an antidote, but with caution due to risk of withdrawal seizures.
Pregnancy & Lactation
Pregnancy Category D. Xanbacine can cause fetal harm. Avoid during pregnancy. Excreted in breast milk, avoid during breastfeeding or monitor infant for sedation/feeding difficulties.
