Overview of the medicine
Xaparin 4000 anti-Xa injection contains Enoxaparin Sodium, a low molecular weight heparin, used to prevent and treat blood clots in various medical conditions.
Uses & Indications
Dosage
Adults
DVT prophylaxis: 20-40 mg (2000-4000 anti-Xa IU) subcutaneously once daily. DVT/PE treatment: 1 mg/kg (100 anti-Xa IU/kg) subcutaneously every 12 hours.
Elderly
No dose adjustment generally required unless severe renal impairment. Careful monitoring is advised.
Renal_impairment
Dosage adjustment required for severe renal impairment (CrCl < 30 mL/min). For prophylaxis, 20 mg once daily.
How to Take
Administer by subcutaneous injection, typically into the anterolateral or posterolateral abdominal wall, or outer aspect of the thigh. Do not inject intramuscularly. Alternate injection sites.
Mechanism of Action
Binds to antithrombin, accelerating its inhibition of Factor Xa and to a lesser extent Factor IIa (thrombin), preventing clot formation.

- Spinal or epidural anesthesia in patients on therapeutic enoxaparin
