Description
Overview of the medicine
Xcel-Max 325 mg Tablet contains Paracetamol (Acetaminophen), a widely used medication for relieving mild to moderate pain and reducing fever. It is effective for conditions like headaches, muscle aches, toothaches, backaches, menstrual periods, and the common cold.
Uses & Indications
Dosage
Adults
Oral: 325 mg to 650 mg every 4-6 hours as needed. Maximum dose: 4000 mg in 24 hours.
Elderly
No specific dose adjustment typically required, but caution with liver/kidney impairment.
Renal_impairment
Dose interval may need to be extended in severe renal impairment (e.g., every 8 hours for CrCl <10 mL/min).
How to Take
Take orally with or without food. Do not exceed the recommended dose.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, increasing the pain threshold by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which are involved in prostaglandin synthesis. Its antipyretic effect is due to direct action on the heat-regulating centers in the hypothalamus, leading to peripheral vasodilation and increased sweating.
Pharmacokinetics
Onset
Pain relief: 10-30 minutes; Fever reduction: within 30 minutes.
Excretion
Excreted mainly in the urine as glucuronide and sulfate conjugates, with less than 5% excreted as unchanged drug.
Half life
1.25 to 3 hours (increased in liver disease).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30 minutes to 2 hours after oral administration.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation, with a small amount undergoing cytochrome P450-mediated oxidation to a toxic intermediate (NAPQI) which is rapidly detoxified by glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation
- Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Increases the risk of hepatotoxicity.
Cholestyramine
Reduces paracetamol absorption if taken within one hour.
Warfarin and other anticoagulants
May enhance the anticoagulant effect, requiring closer monitoring of INR.
Enzyme inducers (e.g., barbiturates, carbamazepine)
May increase the risk of hepatotoxicity by increasing the formation of the toxic metabolite.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, and liver damage (which may be delayed for 24-48 hours). Treatment involves prompt administration of N-acetylcysteine, which is most effective if given within 8 hours of ingestion.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended doses, but consult a doctor before use.

YesClinical Trials
As a well-established and widely used drug, paracetamol has undergone extensive clinical trials demonstrating its efficacy and safety for pain and fever relief. Ongoing research focuses on specific populations or new formulations.
Lab Monitoring
- Liver function tests (ALT, AST, bilirubin) in cases of overdose or prolonged high-dose use.
- Prothrombin time/INR if concomitant warfarin use.
Doctor Notes
- Advise patients not to exceed the maximum daily dose to prevent hepatotoxicity.
- Caution in patients with pre-existing liver or kidney disease.
- Consider drug interactions, especially with warfarin and alcohol.
Patient Guidelines
- Do not exceed the recommended dose.
- Do not take with other paracetamol-containing products.
- Consult a doctor if pain or fever persists for more than a few days.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
