Description
Overview of the medicine
Xclor 125 mg Suspension is an antibiotic from the cephalosporin group, used to treat various bacterial infections like respiratory tract infections, ear infections, skin and soft tissue infections, and urinary tract infections.
Uses & Indications
Dosage
Adults
Usually 250 mg every 8 hours, or 500 mg every 8 hours for more severe infections. Max 4 g/day. (Note: 125 mg suspension is typically for pediatric use, but general adult information is provided).
Elderly
No specific dose adjustment for elderly patients with normal renal function, but monitor renal function.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (creatinine clearance < 10 mL/min). Administer the usual initial dose, then reduce subsequent doses.
How to Take
Shake well before use. May be taken with or without food. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Cefaclor inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Onset of antibacterial action typically within 30-60 minutes.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion.
Half life
Elimination half-life is approximately 0.5 to 1 hour in individuals with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are generally achieved within 0.5 to 1 hour after administration.
Metabolism
Cefaclor is minimally metabolized in the body; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefaclor or any other cephalosporin antibiotics
- History of severe hypersensitivity to penicillin antibiotics
Drug Interactions
Metformin
Potential for reduced metformin excretion, though clinically significant interactions are rare.
Probenecid
Increases cefaclor plasma levels and prolongs its half-life by inhibiting renal excretion.
Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, requiring closer monitoring of INR.
Storage
Store dry powder below 30°C. Keep away from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and discard unused portion after 14 days.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management includes supportive care and activated charcoal if ingestion is recent. Hemodialysis may be useful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Cefaclor should be used during pregnancy only if clearly needed. Small amounts are excreted in breast milk; caution should be exercised when administered to a nursing mother.
