Description
Overview of the medicine
Xelosil is an oral antibiotic belonging to the third-generation cephalosporin class, used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
200 mg to 400 mg once daily, or 200 mg twice daily, depending on the severity of infection.
Elderly
No dose adjustment generally required unless renal function is severely impaired.
Renal_impairment
For creatinine clearance less than 60 mL/min, dosage adjustment is necessary. 200 mg once daily for CrCl 21-60 mL/min, and 100 mg once daily for CrCl less than 20 mL/min.
How to Take
Can be taken with or without food. For best absorption, it is often recommended to take it with food.
Mechanism of Action
Cefixime, the active ingredient in Xelosil, works by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death.

Onset
Peak plasma concentrations are achieved 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%).
Half life
Average half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed, food may delay absorption but not significantly affect the extent.
Metabolism
Approximately 10-15% metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity to penicillin antibiotics.
Drug Interactions
Warfarin
May increase prothrombin time; monitor INR closely.
Carbamazepine
May increase carbamazepine plasma concentrations.
Oral Contraceptives
May reduce efficacy of oral contraceptives (rarely).
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
There is no specific antidote for cefixime overdose. Gastric lavage may be indicated. Symptomatic and supportive treatment is recommended. Hemodialysis or peritoneal dialysis is not substantially effective.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Small amounts are excreted in breast milk; use with caution during lactation.
