Description
Overview of the medicine
Xentoin 100 mg Tablet is an anticonvulsant medicine used to treat various types of seizures, including generalized tonic-clonic and partial seizures. It works by stabilizing electrical activity in the brain.
Uses & Indications
Dosage
Adults
Initial 100 mg three times daily, adjusted according to plasma levels and clinical response. Maintenance dose typically 300-400 mg/day in divided doses. Max 600 mg/day.
Elderly
Lower initial doses (e.g., 50 mg three times daily) and slower titration to minimize side effects.
Renal_impairment
No specific dose adjustment for mild to moderate impairment; monitor for increased side effects. Use with caution in severe impairment.
How to Take
Orally, preferably with meals to minimize gastrointestinal upset. Take at regular intervals each day.
Mechanism of Action
Stabilizes neuronal membranes and prevents the rapid spread of seizure activity by reducing repetitive firing of action potentials. It prolongs the inactive state of voltage-gated sodium channels.
Pharmacokinetics
Onset
Oral: 30-60 minutes; IV: Immediate
Excretion
Mainly renal excretion of metabolites, small amount unchanged in urine and bile.
Half life
6-24 hours (dose-dependent and individualized)
Absorption
Well absorbed from the gastrointestinal tract; bioavailability can be variable.
Metabolism
Primarily hepatic metabolism via CYP2C9 and CYP2C19 enzymes to inactive metabolites.
Side Effects
Contraindications

Cimetidine, Omeprazole

Carbamazepine, Phenobarbital
May decrease Xentoin levels due to enzyme induction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nystagmus, ataxia, slurred speech, tremor, lethargy, blurred vision, nausea, vomiting, and in severe cases, coma and respiratory depression. Management is supportive, including gastric lavage or activated charcoal, and monitoring vital signs.
