Description
Overview of the medicine
Xinarox-CV 500 mg Tablet is a combination antibiotic containing Cefuroxime and Clavulanic Acid. It is used to treat a wide range of bacterial infections, including those caused by beta-lactamase producing strains.
Uses & Indications
Dosage
Adults
One tablet (Cefuroxime 500 mg/Clavulanic Acid 125 mg) twice daily, or as directed by the physician. Duration of treatment varies depending on the infection.
Elderly
Similar to adult dosage, but renal function should be considered and dosage adjusted if necessary.
Renal_impairment
Dosage adjustment is required based on creatinine clearance. For patients with creatinine clearance <30 mL/min, dosage intervals should be extended.
How to Take
Take orally, preferably with food to optimize absorption. Tablets should not be crushed or chewed.
Mechanism of Action
Cefuroxime inhibits bacterial cell wall synthesis. Clavulanic acid is a beta-lactamase inhibitor that protects cefuroxime from degradation by beta-lactamase enzymes, thereby extending its antibacterial spectrum.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 1-3 hours after oral administration.
Excretion
Cefuroxime is excreted unchanged primarily by the kidneys via glomerular filtration and tubular secretion. Clavulanate is excreted via urine, mainly as metabolites, and some unchanged drug.
Half life
The plasma half-life for cefuroxime is approximately 1.2-1.5 hours and for clavulanate is approximately 1 hour.
Absorption
Cefuroxime axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed in the intestinal mucosa and blood to release cefuroxime. Clavulanate is also well absorbed orally.
Metabolism
Cefuroxime is not significantly metabolized in the body. Clavulanate is extensively metabolized.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, clavulanic acid, or any component of the formulation.
- Patients with a history of severe hypersensitivity (e.g., anaphylaxis) to any other beta-lactam antibacterial agents (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Probenecid
Concomitant use with probenecid increases the area under the serum concentration versus time curve of cefuroxime.
Aminoglycosides
Although uncommon, concurrent use with aminoglycosides may increase the risk of nephrotoxicity.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (though clinical significance is generally not established for most antibiotics).
Antacids/H2 blockers/PPIs
Drugs that reduce gastric acidity may result in lower bioavailability of cefuroxime axetil and should be avoided or administered several hours apart.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms include gastrointestinal disturbances and convulsions. Management is supportive and symptomatic. Hemodialysis can reduce serum levels of cefuroxime and clavulanic acid.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use in pregnancy only if clearly needed. Cefuroxime and clavulanic acid are excreted in breast milk in small amounts; use with caution in lactating women, monitoring for infant side effects like diarrhea or candidiasis.
