Description
Overview of the medicine
Moxifloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including respiratory tract, skin, and intra-abdominal infections.
Uses & Indications
Dosage
Adults
400 mg orally once daily for 5-21 days, depending on the type and severity of infection.
Elderly
No specific dosage adjustment required based solely on age, but renal function should be monitored.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (CrCl ≥30 mL/min). For severe impairment, use with caution; consult physician.
How to Take
Take orally with or without food. Do not take with antacids containing magnesium or aluminum, multivitamins, or iron/zinc supplements; separate by several hours.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Approximately 50% renally (20% unchanged, 30% metabolites) and 50% hepatically/biliary.
Half life
Approximately 11-15 hours
Absorption
Rapid and well-absorbed after oral administration; absolute bioavailability is approximately 90%. Peak plasma concentrations occur 0.5-4 hours post-dose.
Metabolism
Primarily hepatic, with conjugation to sulfate and glucuronide. Not metabolized by cytochrome P450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolone antibiotics.
- History of tendinitis or tendon rupture associated with quinolone use.
- Children and adolescents below 18 years of age.
- Pregnancy and lactation (due to potential risks).
- Patients with known QTc prolongation, uncorrected hypokalemia, or significant bradycardia.
Drug Interactions
Warfarin
May enhance the anticoagulant effects, requiring increased INR monitoring.
Corticosteroids
Increased risk of tendon rupture.
Antiarrhythmics (Class IA and III)
Increased risk of QTc prolongation; avoid concomitant use.
Antacids (Mg/Al), Sucralfate, Iron/Zinc supplements
May decrease oral absorption of moxifloxacin. Separate administration by several hours.
Drugs that prolong QTc interval (e.g., cisapride, erythromycin, antipsychotics)
Increased risk of serious cardiac arrhythmias.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include CNS effects (e.g., seizures), QTc prolongation, and gastrointestinal disturbances. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered. Hemodialysis or peritoneal dialysis is not effective in removing moxifloxacin.
Pregnancy & Lactation
Moxifloxacin is contraindicated during pregnancy and lactation due to potential adverse effects on the fetus/infant, including cartilage damage. Use only if potential benefits outweigh risks, and under strict medical supervision.

Lab Monitoring
- Renal function tests (especially in elderly or those with existing impairment)
- Liver function tests (baseline and during therapy if indicated)
- ECG (before and during treatment in patients at risk for QTc prolongation)
- Blood glucose levels (in diabetic patients due to risk of dysglycemia)
Doctor Notes
- Emphasize the black box warnings to patients, especially concerning tendinopathy, tendon rupture, and neurological adverse events.
- Avoid concomitant administration with other drugs that prolong the QTc interval.
- Advise patients to immediately discontinue the drug and seek medical attention if they experience any signs of tendon issues or neurological symptoms.
- Consider alternative antibiotics for uncomplicated infections where a fluoroquinolone might not be the first choice.
Patient Guidelines
- Complete the full prescribed course of treatment, even if symptoms improve.
- Avoid excessive sun exposure or artificial UV light, as it may cause photosensitivity.
- Report any joint pain, swelling, numbness, tingling, or weakness immediately.
- Do not take with antacids, iron, or zinc supplements; maintain a separation of several hours.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Moxifloxacin may cause dizziness, lightheadedness, or visual disturbances. Patients should be advised not to drive or operate machinery if they experience these effects.
Lifestyle Advice
- Stay well-hydrated during the course of treatment.
- Avoid strenuous physical activity that could stress tendons.
- Monitor blood sugar levels regularly if you are diabetic.
