Description
Overview of the medicine
Tobramycin is an aminoglycoside antibiotic used to treat bacterial infections of the eye. It is effective against a wide range of gram-negative and some gram-positive bacteria.
Uses & Indications
Dosage
Adults
Instill 1 or 2 drops into the affected eye(s) every 4 hours. For severe infections, instill 2 drops every hour until improvement, then reduce frequency.
Elderly
No specific dosage adjustment required.
Renal_impairment
No dosage adjustment required for ophthalmic use due to minimal systemic absorption.
How to Take
For ophthalmic use only. Tilt head back, pull down lower eyelid, and instill drops into the conjunctival sac. Avoid touching the dropper tip to the eye or any other surface.
Mechanism of Action
Tobramycin binds to the 30S ribosomal subunit, inhibiting bacterial protein synthesis and leading to bacterial cell death.
Pharmacokinetics
Onset
Rapid, usually within minutes for bacterial inhibition.
Excretion
Primarily excreted unchanged in urine if absorbed systemically.
Half life
Not well-defined for ophthalmic use due to minimal systemic absorption.
Absorption
Minimal systemic absorption from ophthalmic administration.
Metabolism
Not significantly metabolized in the eye; minimal systemic metabolism.
Side Effects
Contraindications
- Hypersensitivity to tobramycin or any component of the formulation.
Drug Interactions
Systemic aminoglycosides
Although minimal systemic absorption, caution with concurrent systemic aminoglycoside use due to potential additive toxicity.
Other ophthalmic medications
If using other eye drops, wait at least 5 minutes between applications.
Storage
Store at room temperature (15-30°C). Protect from light and moisture. Do not freeze.
Overdose
Local ophthalmic overdose can be flushed from the eye with lukewarm water. Systemic overdose is unlikely due to minimal absorption.
Pregnancy & Lactation
Use during pregnancy only if clearly needed. Exercise caution when administering to a nursing woman.
