Description
Overview of the medicine
Xoferon 100 mg Injection is an intravenous iron preparation containing iron sucrose, used for the treatment of iron deficiency anemia in patients undergoing hemodialysis who are receiving erythropoietin therapy, and for non-dialysis dependent chronic kidney disease patients.
Uses & Indications
Dosage
Adults
Usually 100 mg (5 mL) administered intravenously as a slow injection over 2-5 minutes or infused over 15-30 minutes, 1-3 times a week, up to a cumulative dose of 1000 mg (10 doses). Dose and frequency adjusted based on patient's iron status and response.
Elderly
No specific dose adjustment required for elderly patients, but caution is advised due to potential for comorbidities and polypharmacy.
Renal_impairment
Used in patients with chronic kidney disease; dosage is as specified in indications.
How to Take
Administered intravenously, either by slow injection (over 2-5 minutes) or by infusion (diluted in normal saline, over 15-30 minutes or longer for higher doses). Must not be administered intramuscularly or subcutaneously.
Mechanism of Action
Iron sucrose complex is dissociated by the reticuloendothelial system into iron and sucrose. Iron is then transported by transferrin to the bone marrow for erythropoiesis, or stored in the liver as ferritin. It replenishes iron stores necessary for hemoglobin synthesis.
Pharmacokinetics
Onset
Hemoglobin response usually observed within weeks.
Excretion
Renal excretion of sucrose and minimal iron; iron is mostly utilized or stored.
Half life
The iron component has a half-life of 3-4 hours after administration. The sucrose component is rapidly eliminated.
Absorption
Administered intravenously, so bioavailability is 100%. Iron is taken up by reticuloendothelial cells.
Metabolism
Iron is metabolized within the reticuloendothelial system. Sucrose is largely cleared.
Side Effects
Contraindications
- Hypersensitivity to iron sucrose or any of its components.
- Anemia not caused by iron deficiency.
- Iron overload or disturbances in iron utilization (e.g., hemochromatosis, hemosiderosis).
- First trimester of pregnancy.
Drug Interactions
Oral iron preparations
Reduced absorption of oral iron preparations when administered concomitantly.
Angiotensin-converting enzyme (ACE) inhibitors
Increased risk of hypotension with concomitant use in some cases.
Storage
Store at controlled room temperature (20-25°C), excursions permitted to 15-30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Overdose can lead to acute iron overload, which may manifest as hypotension, collapse, nausea, vomiting, abdominal pain, diarrhea. Long-term overdose can lead to hemosiderosis. Treatment involves supportive care and iron chelating agents (e.g., deferoxamine) if severe.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Avoid during the first trimester. Limited data on excretion in breast milk; caution should be exercised when administered to a nursing woman.
