Uses & Indications
Dosage
Adults
Initial dose 0.25-0.5 mg three times daily. May be increased up to 4 mg daily in divided doses based on response.
Elderly
Initial dose 0.25 mg two to three times daily. Dose adjustments should be made cautiously due to increased sensitivity.
Renal_impairment
No specific dose adjustment guidelines; use with caution. Consider lower starting doses.
How to Take
Take orally with or without food. Do not crush, chew, or break the tablet. Swallow whole.
Mechanism of Action

Drug Interactions
Opioids

Potent CYP3A4 inhibitors, significantly increase Xolmel plasma concentrations.
Alcohol and other CNS depressants
Additive CNS depressant effects, increasing risk of drowsiness and sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, impaired coordination, diminished reflexes, and coma. Management is primarily supportive; maintain airway, monitor vital signs. Flumazenil may be considered in severe cases, but caution is advised due to potential for withdrawal symptoms or seizures.
Pregnancy & Lactation
Xolmel is not recommended during pregnancy, especially during the first trimester, due to potential risk of congenital malformations. It is excreted in breast milk and may cause sedation in the infant; therefore, use is generally not recommended during lactation.
