Description
Overview of the medicine
Xten 20 mg Tablet contains Teneligliptin, an oral antidiabetic drug used to treat type 2 diabetes mellitus. It helps to improve glycemic control by increasing insulin secretion and decreasing glucagon secretion.
Uses & Indications
Dosage
Adults
The recommended dose is 20 mg once daily. If needed, the dose can be increased to 40 mg once daily based on glycemic control and tolerance.
Elderly
No dosage adjustment is generally required based on age alone, but renal function should be considered.
Renal_impairment
No dosage adjustment is required for mild to severe renal impairment. For patients with end-stage renal disease on dialysis, consider 10-20 mg once daily as advised by a physician.
How to Take
Xten 20 mg Tablet should be taken orally once daily, with or without food, at any time of the day.
Mechanism of Action
Teneligliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor. It prevents the degradation of incretin hormones (GLP-1 and GIP), thereby increasing their levels. This leads to glucose-dependent enhancement of insulin secretion from pancreatic beta cells and suppression of glucagon secretion from alpha cells, resulting in improved glycemic control.
Pharmacokinetics
Onset
Within 1-2 hours.
Excretion
Excreted through both urine (approximately 33-57%) and feces (approximately 34-55%).
Half life
Approximately 24.2 hours (terminal half-life), allowing for once-daily dosing.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentration within 1-2 hours.
Metabolism
Primarily metabolized by cytochrome P450 3A4 (CYP3A4) and flavin-containing monooxygenase 3 (FMO3) in the liver.
Side Effects
Contraindications
- Hypersensitivity to Teneligliptin or any component of the tablet.
- Type 1 diabetes mellitus.
- Diabetic ketoacidosis.
Drug Interactions
Digoxin
May slightly increase Digoxin levels; monitor Digoxin concentrations if co-administered.
Other antidiabetic agents (e.g., sulfonylureas, insulin)
Increased risk of hypoglycemia when co-administered. Dose adjustment of these agents may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive treatment should be initiated as needed. Teneligliptin is not effectively removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy and lactation only if clearly needed and the potential benefits outweigh the potential risks. Consult your doctor.

- Renal function tests (e.g., creatinine, eGFR)
Doctor Notes
- Advise patients on the importance of diet and exercise in diabetes management.
- Educate patients about the signs and symptoms of hypoglycemia, especially when Teneligliptin is combined with sulfonylureas or insulin.

- Take the medicine exactly as prescribed by your doctor.
- Maintain a healthy diet and engage in regular physical activity.
- Monitor your blood glucose levels regularly as advised.
- Do not stop taking the medicine without consulting your doctor.
Missed Dose Advice
