Overview of the medicine
Xtracal-D is a combination medication containing Calcium Carbonate and Vitamin D3, primarily used to treat and prevent calcium and vitamin D deficiencies, and for maintaining bone health.
Uses & Indications
Dosage
Adults
1-2 tablets daily, or as directed by physician.
Elderly
Same as adult dosage, or as directed by physician.
Renal_impairment
Use with caution, monitor calcium levels. Dosage adjustment may be required.
How to Take
Oral. Can be taken with or without food, but generally advised with food for better absorption and to reduce gastrointestinal upset.
Mechanism of Action
Calcium carbonate provides elemental calcium essential for bone formation, muscle contraction, nerve function, and blood coagulation. Vitamin D3 (Cholecalciferol) helps in the absorption of calcium from the gut and its deposition in bones.
Pharmacokinetics
Onset
Gradual, for bone health benefits over weeks/months.
Excretion
Calcium is excreted via kidneys and feces. Vitamin D metabolites are excreted primarily in bile/feces.
Half life
Calcium has no true half-life. Vitamin D3 half-life is about 24 hours, but calcifediol (25-OH-D) has a half-life of 2-3 weeks.
Absorption
Calcium is absorbed primarily in the small intestine (15-30%), enhanced by Vitamin D. Vitamin D3 is absorbed from the small intestine and requires bile salts.
Metabolism
Calcium is not metabolized. Vitamin D3 is metabolized in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol).

Calcium can reduce absorption, take 2-4 hours apart.
Tetracycline and Fluoroquinolone antibiotics
Calcium can reduce absorption, take 2-4 hours apart.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms include hypercalcaemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, fatigue), hypercalciuria. Management involves stopping treatment, rehydration, and in severe cases, diuretics or corticosteroids.
