Description
Overview of the medicine
Rabeprazole is a proton pump inhibitor used to treat conditions involving excessive stomach acid, such as heartburn, acid reflux, gastroesophageal reflux disease (GERD), and peptic ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For duodenal ulcer: 20 mg once daily for 4 weeks. For H. pylori eradication: 20 mg twice daily with two antibiotics for 7 days.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is generally required for patients with mild to moderate renal impairment. Caution is advised in severe impairment.
How to Take
Zedex-20mg tablet should be swallowed whole, preferably in the morning before a meal. Do not crush, chew, or split the tablet.
Mechanism of Action
Rabeprazole irreversibly binds to and inhibits the H+/K+-ATPase pump (proton pump) in gastric parietal cells, leading to a significant reduction in gastric acid secretion.
Pharmacokinetics
Onset
Acid inhibitory effect starts within 1 hour, with maximal effect seen within 2-4 hours.
Excretion
Approximately 90% of a dose is excreted via urine (as metabolites) and the remainder in feces.
Half life
Plasma elimination half-life is 1-2 hours, but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-5 hours. Bioavailability is approximately 52% after the first dose.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes. Metabolites are mostly inactive.
Side Effects
Contraindications
- Hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
- Pregnancy and lactation (unless clearly indicated by a physician).
Drug Interactions
Warfarin
There have been reports of increased INR and prothrombin time in patients receiving concomitant PPIs and warfarin, which may lead to abnormal bleeding.
Methotrexate
Concomitant use with high-dose methotrexate may increase and prolong serum levels of methotrexate and its metabolite, potentially leading to toxicity.
Drugs dependent on gastric pH for absorption (e.g., Ketoconazole, Itraconazole, Atazanavir, Iron salts)
Rabeprazole can reduce the absorption of these drugs due to its acid-suppressing effects.

Experience with rabeprazole overdose is limited. Symptomatic and supportive treatment should be given in case of overdose. Rabeprazole is not readily dialyzable.

- Avoid trigger foods like spicy foods, fatty foods, chocolate, caffeine, and acidic beverages if they worsen your symptoms.
