Description
Overview of the medicine
Levosulpiride is a substituted benzamide derivative with selective dopamine D2 receptor antagonistic activity. It is used to treat gastrointestinal motility disorders like functional dyspepsia and nausea/vomiting.
Uses & Indications
Dosage
Adults
20 mg (10 ml) three times daily before meals. Duration of treatment depends on clinical response.
Elderly
Similar to adult dosage, but use with caution due to potential for increased sensitivity.
Renal_impairment
Dose reduction is recommended based on creatinine clearance (e.g., reduce dose by 50% for CrCl 30-60 ml/min, 70% for CrCl <30 ml/min).
How to Take
Zemin-DS syrup should be taken orally, preferably 15-30 minutes before meals, as directed by the physician. Use the provided measuring cup for accurate dosing.
Mechanism of Action
Levosulpiride selectively blocks dopamine D2 receptors in the chemoreceptor trigger zone and peripherally in the gastrointestinal tract. This blockade enhances acetylcholine release, thereby improving gastric motility and emptying and exerting antiemetic effects.
Pharmacokinetics
Onset
Within 30-60 minutes for gastrointestinal effects.
Excretion
Primarily excreted unchanged via the kidneys in urine.
Half life
Approximately 6-8 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached within 1-3 hours.
Metabolism
Minimally metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to levosulpiride or any component of the formulation
- Pheochromocytoma (risk of hypertensive crisis)
- Epilepsy or other convulsive states
- Prolactin-dependent tumors (e.g., pituitary adenoma, breast cancer)
- Gastrointestinal hemorrhage, mechanical obstruction, or perforation
Drug Interactions
Alcohol
May enhance sedative effects.
Antacids
May reduce absorption of levosulpiride if taken simultaneously; administer at least 2 hours apart.
Dopaminergic agonists (e.g., Levodopa)
Antagonistic effects, may reduce efficacy of levodopa.
CNS depressants (e.g., benzodiazepines, opioids)
Increased risk of sedation.
QT-prolonging drugs (e.g., antiarrhythmics, some antidepressants)
Increased risk of cardiac arrhythmias.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose may include drowsiness, extrapyramidal symptoms (e.g., tremors, rigidity), hypotension, and coma. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal.
Pregnancy & Lactation
Avoid use during pregnancy and lactation unless the potential benefits outweigh the potential risks. Consult a doctor before use.

Clinical Trials
Clinical trials have shown levosulpiride to be effective in treating functional dyspepsia and improving gastric motility. Ongoing research explores its broader applications in GI disorders.
Lab Monitoring
- Prolactin levels (especially if symptoms of hyperprolactinemia occur, such as galactorrhea or menstrual irregularities)
- Renal function (creatinine clearance) in patients with kidney impairment for dose adjustment
Doctor Notes
- Assess for potential hyperprolactinemia symptoms, especially in female patients, and consider baseline prolactin levels for long-term therapy.
- Monitor for extrapyramidal symptoms; adjust dose or discontinue if severe.
- Careful dose titration is necessary in patients with renal impairment to prevent accumulation.
- Avoid concomitant use with other QT-prolonging drugs.
Patient Guidelines
- Take the medicine as directed by your doctor, preferably before meals.
- Do not exceed the recommended dose.
- Report any unusual side effects, especially breast changes or menstrual irregularities.
- Avoid alcohol while on this medication.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Zemin-DS may cause drowsiness or dizziness, especially at the beginning of treatment. Patients should be advised not to drive or operate machinery if they experience these effects.
Lifestyle Advice
- Maintain a healthy diet and avoid trigger foods that worsen dyspepsia.
- Manage stress through relaxation techniques.
- Engage in regular physical activity.
