Description
Overview of the medicine
Zerodol 100 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used for pain relief and inflammation associated with conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. It works by blocking the production of certain chemical messengers that cause pain and inflammation.
Uses & Indications
Dosage
Adults
100 mg twice daily (every 12 hours).
Elderly
Usually no dose adjustment required, but caution is advised. Lowest effective dose for shortest duration.
Renal_impairment
No significant dose reduction needed for mild renal impairment, but caution in moderate to severe impairment. Start with reduced dose (e.g., 50 mg twice daily) and monitor.
How to Take
Orally, preferably with or after food to minimize gastrointestinal discomfort. Swallow the tablet whole with a glass of water. Do not chew or crush.
Mechanism of Action
Aceclofenac acts primarily by inhibiting prostaglandin synthesis through competitive inhibition of cyclooxygenase (COX) enzymes, COX-1 and COX-2. It exhibits greater selectivity for COX-2, leading to anti-inflammatory, analgesic, and antipyretic effects.
Pharmacokinetics
Onset
Analgesic effect within 30-60 minutes.
Excretion
Mainly excreted via urine (approx. 2/3) and feces (approx. 1/3) as metabolites.
Half life
Approximately 4 hours.
Absorption
Rapidly and completely absorbed after oral administration, peak plasma concentrations reached in 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to 4'-hydroxyaceclofenac.

Symptoms may include nausea, vomiting, epigastric pain, gastrointestinal bleeding, diarrhea, dizziness, disorientation, excitation, tinnitus, convulsions. In case of overdose, symptomatic and supportive treatment should be given. Gastric lavage and activated charcoal may be considered within one hour of ingestion.
