How to Take
Zest 6 mg tablet is a chewable tablet and should be chewed thoroughly before swallowing. It is usually taken once daily in the evening, with or without food. For asthma, it should be taken in the evening. For allergic rhinitis, the time of administration may be individualized.
Mechanism of Action
Montelukast selectively antagonizes the cysteinyl leukotriene 1 (CysLT1) receptor. Leukotrienes are potent inflammatory mediators released from various cells, including mast cells and eosinophils. By blocking CysLT1 receptors, Montelukast inhibits the bronchoconstriction and inflammatory effects of leukotrienes, thereby improving asthma symptoms and allergic rhinitis.
Pharmacokinetics
Onset
Within one day for asthma control.
Excretion
Primarily excreted in feces via bile (approximately 86%), with minimal renal excretion.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration is achieved in 2-4 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP3A4, CYP2C9, CYP2A6).
Side Effects
Contraindications
- Hypersensitivity to Montelukast or any component of the formulation.
- Not indicated for the treatment of acute asthma attacks.
Drug Interactions
Rifampicin
May decrease Montelukast plasma concentration due to induction of CYP3A4, CYP2C9, and CYP2A6.
Phenobarbital
May decrease Montelukast plasma concentration.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, common adverse events included abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is unknown whether Montelukast is excreted in human milk, so caution should be exercised when administered to a nursing mother.
