Description
Overview of the medicine
Bilastine is a non-sedating, long-acting H1-receptor antagonist used for the symptomatic treatment of allergic rhinoconjunctivitis (seasonal and perennial) and urticaria.
Uses & Indications
Dosage
Adults
20 mg once daily.
Elderly
No dose adjustment required.
Renal_impairment
No dose adjustment generally required. Caution is advised in patients with moderate to severe renal impairment (creatinine clearance <50 mL/min) who are concomitantly receiving P-gp inhibitors (e.g., ketoconazole, erythromycin, cyclosporine, ritonavir, diltiazem).
How to Take
Take the tablet orally, preferably on an empty stomach, one hour before or two hours after food or fruit juice. Swallow the tablet with a glass of water.
Mechanism of Action
Bilastine is a selective peripheral H1-receptor antagonist. It prevents the effects of histamine by competitively binding to H1 receptors, thereby alleviating symptoms of allergic reactions without significant central nervous system effects.

Within 30-60 minutes
Excretion
Primarily excreted unchanged, with approximately 33% in urine and 67% in feces.
Half life
Approximately 14.5 hours
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached in approximately 1.3 hours. Bioavailability is around 61% and is reduced by food intake.
Metabolism
Bilastine is not significantly metabolized in humans and does not induce or inhibit CYP450 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to the active substance bilastine or to any of the excipients.
- •Patients below 12 years of age (due to lack of data for 20mg formulation).
Drug Interactions
Diltiazem
Concomitant use with diltiazem (a P-glycoprotein inhibitor) increases bilastine plasma concentrations. This interaction does not appear to increase the risk of adverse effects.
Erythromycin
Concomitant use with erythromycin (a P-glycoprotein inhibitor) increases bilastine plasma concentrations. This interaction does not appear to increase the risk of adverse effects.
Ketoconazole
Concomitant use with ketoconazole (a P-glycoprotein inhibitor) increases bilastine plasma concentrations. This interaction does not appear to increase the risk of adverse effects.
Ritonavir/Cyclosporine
Other P-gp inhibitors like ritonavir or cyclosporine may also increase bilastine plasma levels. Use with caution in patients with moderate to severe renal impairment when co-administered with P-gp inhibitors.
Storage
Store below 30°C, in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no known specific antidote to bilastine. Monitor vital signs and observe for adverse reactions.
Pregnancy & Lactation
Limited data on use in pregnant women. Animal studies do not indicate direct or indirect harmful effects on reproductive toxicity. As a precautionary measure, it is preferable to avoid the use of Bilastine during pregnancy and lactation. Consult a doctor.
