Description
Overview of the medicine
Zilic-TR 150 mg Capsule contains Itopride Hydrochloride as a sustained-release formulation. It is used to treat gastrointestinal symptoms associated with functional dyspepsia and other motility disorders.
Uses & Indications
Dosage
Adults
One 150 mg capsule once daily. Should be taken before meals.
Elderly
No specific dose adjustment is usually required, but caution is advised due to potential age-related physiological decline.
Renal_impairment
Use with caution in patients with renal impairment. Dose reduction may be considered based on clinical assessment.
How to Take
Take orally, preferably before meals, with a glass of water. Do not chew or crush the capsule.
Mechanism of Action
Itopride enhances gastric motility by acting as a dopamine D2 receptor antagonist and an acetylcholinesterase inhibitor. This dual mechanism increases acetylcholine concentration, promoting gastrointestinal prokinetic effects and accelerating gastric emptying.
Pharmacokinetics
Onset
Onset of action typically within 30-60 minutes for immediate release; sustained release provides a prolonged effect.
Excretion
Mainly excreted via urine, with some fecal excretion.
Half life
Approximately 6 hours for Itopride; TR formulation extends effective duration.
Absorption
Rapidly and well absorbed from the GI tract. Bioavailability is approximately 60% after oral administration.
Metabolism
Extensively metabolized in the liver, primarily by flavin-containing monooxygenase (FMO3).
Side Effects
Contraindications
- Known hypersensitivity to Itopride or any component of the formulation.
- Patients with gastrointestinal hemorrhage, mechanical obstruction, or perforation.
- Patients with prolactinoma due to its dopamine D2 antagonist activity.
Drug Interactions
Dopamine agonists
Itopride may counteract the effects of dopamine agonists (e.g., bromocriptine, cabergoline) due to its D2 antagonist activity.
Anticholinergic drugs
Anticholinergic drugs (e.g., atropine, dicyclomine) may decrease the prokinetic effect of Itopride. Avoid concomitant use.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage and symptomatic supportive treatment are recommended. Monitor for extrapyramidal symptoms.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the risks. Itopride is excreted into breast milk; breastfeeding is not recommended.
