Uses & Indications
Dosage
Adults
For zinc deficiency: Consult physician. Typically 20-40 mg elemental zinc daily in divided doses. This 10mg syrup strength is not commonly used for adults.
Elderly
No specific dose adjustment is generally required, but caution is advised in individuals with pre-existing renal impairment.
Children
Acute diarrhea (6 months to 5 years): 20 mg elemental zinc (10 ml of 10 mg/5ml syrup) once daily for 10-14 days. Acute diarrhea (under 6 months): 10 mg elemental zinc (5 ml of 10 mg/5ml syrup) once daily for 10-14 days. For zinc deficiency: Consult physician. Dosage varies by age and severity.
Renal_impairment
Use with caution. Dosage adjustment may be required in severe renal impairment. Consult physician.
How to Take
Oral administration. Can be taken with or without food. To reduce gastric irritation, it may be taken after meals. Use a measuring spoon/cup for accurate dosing.
Mechanism of Action
Zinc is an essential trace element vital for numerous biological processes including immune function, wound healing, DNA synthesis, and enzymatic reactions. It plays a crucial role in maintaining cell membrane integrity and has antioxidant properties. In diarrhea, it helps in regeneration of gut mucosa, enhances water and electrolyte absorption, and boosts immune response.
Pharmacokinetics
Onset
Therapeutic effects, especially in diarrhea, may be observed within a few days of starting treatment.
Excretion
Primarily via feces (about 90%) and to a lesser extent via urine and sweat.
Half life
Not clearly defined for elemental zinc, as it's widely distributed and utilized. The biological half-life can range from 100 to 300 days depending on various factors.
Absorption
Approximately 20-30% of ingested zinc is absorbed from the small intestine, primarily duodenum and jejunum. Absorption is influenced by dietary factors and other mineral intake.
Metabolism
Zinc is not metabolized in the traditional sense; it is incorporated into various proteins and enzymes throughout the body.
Side Effects
Contraindications
- •Hypersensitivity to zinc sulfate or any component of the formulation
Drug Interactions
Penicillamine
Zinc may decrease the absorption of penicillamine.
Iron supplements
High doses of iron can interfere with zinc absorption. Separate administration times.
Calcium & Phytates
High intake of calcium or phytates (found in whole grains, legumes) can inhibit zinc absorption.
Tetracyclines & Quinolones
Zinc can reduce the absorption of tetracyclines and quinolone antibiotics. Administer zinc at least 2 hours before or 4-6 hours after these antibiotics.
Storage
Store below 30°C in a cool, dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, diarrhea, and irritation of the mouth and throat. Chronic overdose can lead to copper deficiency and anemia. Management: Symptomatic and supportive care. Gastric lavage may be considered if recent and severe. Chelation therapy (e.g., EDTA) may be needed in severe cases but is rarely necessary.
Pregnancy & Lactation
Pregnancy: Zinc is essential during pregnancy, but excessive doses should be avoided. Use as directed by a physician. Lactation: Zinc is excreted in breast milk. Generally considered safe at recommended doses, but consult a physician.
