Overview of the medicine
Zispin (Mirtazapine) is an antidepressant primarily used to treat major depressive disorder. It belongs to a class of drugs called noradrenergic and specific serotonergic antidepressants (NaSSA).
Uses & Indications
Dosage
Adults
Initial dose is usually 15 mg once daily, preferably at bedtime. The dose can be gradually increased to a maximum of 45 mg daily.
Elderly
Similar to adults, but cautious titration and careful monitoring are advised due to potential for increased sensitivity.
Renal_impairment
Dose adjustments may be necessary in patients with moderate to severe renal impairment. Start with a lower dose and monitor carefully.
How to Take
Take orally, once daily, preferably in the evening before sleep. Can be taken with or without food.
Mechanism of Action
Mirtazapine enhances noradrenergic and serotonergic neurotransmission in the central nervous system by blocking central pre-synaptic alpha2-adrenergic autoreceptors and heteroreceptors. It also blocks 5-HT2 and 5-HT3 serotonin receptors and H1 histamine receptors.
Pharmacokinetics
Onset
Antidepressant effects usually become apparent within 1-2 weeks. Sedative effects may begin sooner.
Excretion
Primarily excreted in the urine (approx. 75%) and feces (approx. 15%) within a few days.
Half life
Elimination half-life ranges from 20 to 40 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with absolute bioavailability of about 50%. Peak plasma concentrations reached within approximately 2 hours.
Metabolism
Extensively metabolized in the liver by demethylation and hydroxylation, followed by conjugation. CYP2D6, CYP1A2, and CYP3A4 enzymes are involved.
Side Effects
Contraindications
- •Hypersensitivity to Mirtazapine or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
Drug Interactions
MAOIs
Risk of serotonin syndrome. Concomitant use is contraindicated.
CNS Depressants (e.g., alcohol, benzodiazepines)
Increased sedative effects. Avoid concomitant use.
CYP1A2, CYP2D6, CYP3A4 Inducers (e.g., carbamazepine, rifampicin)
May decrease mirtazapine plasma concentrations.
CYP1A2, CYP2D6, CYP3A4 Inhibitors (e.g., cimetidine, ketoconazole)
May increase mirtazapine plasma concentrations.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose include disorientation, drowsiness, impaired memory, tachycardia. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk. Mirtazapine is excreted in breast milk; caution is advised during breastfeeding.
