Description
Overview of the medicine
Zoclast is a bisphosphonate used for the treatment and prevention of osteoporosis, particularly in postmenopausal women. It helps to increase bone mineral density and reduce the risk of fractures by slowing bone loss.
Uses & Indications
Dosage
Adults
For osteoporosis: 70 mg once weekly, or 10 mg once daily. For Paget's disease: 40 mg once daily for six months.
Elderly
No dosage adjustment required.
Renal_impairment
Not recommended for patients with creatinine clearance less than 35 mL/min.
How to Take
Take Zoclast with a full glass of plain water (not mineral water or other beverages) at least 30 minutes before the first food, beverage, or medication of the day. Do not lie down for at least 30 minutes after taking the dose, and remain upright. Do not chew or suck the tablet.
Mechanism of Action
Zoclast selectively inhibits the activity of osteoclasts, the cells responsible for bone resorption. It binds to bone hydroxyapatite crystals, preventing their dissolution and thereby reducing bone breakdown.

Onset
Clinical effects on bone density are typically observed within 3-6 months.
Excretion
Primarily excreted unchanged by the kidneys.
Half life
Terminal half-life is greater than 10 years, as it is retained in the bone.

Side Effects
Contraindications
- Abnormalities of the esophagus which delay esophageal emptying (e.g., stricture or achalasia).
- Inability to stand or sit upright for at least 30 minutes.
- Hypocalcemia.
- Severe renal impairment (creatinine clearance < 35 mL/min).
- Hypersensitivity to any component of the product.
Drug Interactions
NSAIDs
Concomitant use with NSAIDs may increase the risk of gastrointestinal irritation.
Calcium supplements, antacids
May interfere with the absorption of Zoclast. Take Zoclast at least 30 minutes before other oral medications.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose may include hypocalcemia, hypophosphatemia, and upper gastrointestinal adverse events such as upset stomach, heartburn, esophagitis, gastritis, or ulcer. Management involves milk or antacids to bind Zoclast, and symptomatic treatment.
