Description
Overview of the medicine
Zolaren Plus is a combination medicine containing Diclofenac Potassium and Paracetamol, used for rapid relief from pain, inflammation, and fever.
Uses & Indications
Dosage
Adults
One tablet 2-3 times daily, preferably after food.
Elderly
Use with caution; a reduced dosage may be required due to increased risk of side effects.

Use with caution; dose adjustment may be necessary based on renal function.
How to Take
Take orally, preferably after meals to reduce gastrointestinal upset. Do not chew or crush the tablet.
Mechanism of Action
Diclofenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing prostaglandin synthesis. Paracetamol primarily inhibits prostaglandin synthesis in the central nervous system.
Pharmacokinetics
Onset
Diclofenac: 30 minutes to 1 hour. Paracetamol: 30 minutes.
Excretion
Primarily excreted via the kidneys (urine) as metabolites.
Half life
Diclofenac: approximately 1-2 hours. Paracetamol: 2-3 hours.
Absorption
Diclofenac is well absorbed orally, reaching peak plasma levels within 1-2 hours. Paracetamol is rapidly and almost completely absorbed from the GI tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Both primarily undergo hepatic metabolism. Diclofenac undergoes extensive first-pass metabolism. Paracetamol is metabolized mainly by glucuronidation and sulfation.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, paracetamol, or any component of the formulation
- Active peptic ulcer or gastrointestinal bleeding
- Severe hepatic or renal impairment
- Severe heart failure
- Third trimester of pregnancy
- Patients with aspirin-sensitive asthma or other NSAID-induced allergic reactions
Drug Interactions
Lithium and Methotrexate
Increased plasma levels of lithium and methotrexate, leading to potential toxicity.
ACE inhibitors and Diuretics
Reduced antihypertensive and diuretic effects.
Other NSAIDs or corticosteroids
Increased risk of gastrointestinal adverse effects.
Warfarin and other anticoagulants
Increased risk of bleeding.

Symptoms of diclofenac overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Paracetamol overdose can cause hepatic necrosis (liver damage) 24-48 hours after ingestion. Management involves gastric lavage, activated charcoal, and supportive care. For paracetamol overdose, N-acetylcysteine is the antidote.
Pregnancy & Lactation
Diclofenac is contraindicated in the third trimester of pregnancy due to the risk of premature closure of the ductus arteriosus and renal dysfunction in the fetus. Use with caution in the first two trimesters and during lactation, only if the potential benefits outweigh the risks.
