Description
Overview of the medicine
Zoledronic acid is a bisphosphonate medication used to treat and prevent osteoporosis, treat Paget's disease of bone, and prevent new clinical fractures after a low-trauma hip fracture. It works by slowing bone breakdown and increasing bone density.
Uses & Indications
Dosage
Adults
For osteoporosis: 5 mg intravenously once a year. For Paget's disease: 5 mg intravenously as a single dose.
Elderly
No specific dosage adjustment required based on age alone, but renal function should be monitored.
Renal_impairment
Not recommended in patients with creatinine clearance < 35 mL/min. For creatinine clearance 35-60 mL/min, use with caution, consider lower dose or longer interval.
How to Take
Administer as a single intravenous infusion over at least 15 minutes. Ensure adequate hydration before administration.
Mechanism of Action
Zoledronic acid inhibits bone resorption by binding to hydroxyapatite crystals in the bone and acting as an anti-osteoclastic agent. It inhibits the mevalonate pathway in osteoclasts, leading to osteoclast apoptosis and reduced bone turnover.
Pharmacokinetics
Onset
Within 24 hours for hypercalcemia of malignancy; effects for osteoporosis seen over months.
Excretion
Renal excretion (approximately 50% unchanged drug excreted within 24 hours).
Half life
Initial phase: 0.16 hours; terminal phase: up to 146 hours (reflecting bone binding).
Absorption
Rapidly distributed to bone. Negligible systemic absorption after oral administration; administered intravenously.
Metabolism
Not metabolized in humans.
Side Effects
Contraindications

Drug Interactions
Loop diuretics

Storage
Store at room temperature (20°C to 25°C), do not freeze. Protect from light. Administer immediately after dilution if applicable. Keep out of reach of children.
Overdose
Overdose may lead to hypocalcemia, hypophosphatemia, and hypomagnesemia. Treat with intravenous calcium gluconate, phosphorus, and magnesium as appropriate.
Pregnancy & Lactation
Contraindicated in pregnancy and lactation due to potential fetal harm and unknown excretion into breast milk.
