Description
Overview of the medicine
Zolibac (Cefixime) is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the ear, nose, throat, lower respiratory tract, urinary tract, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
Adults and children over 12 years or weighing >50 kg: 200-400 mg once daily or 200 mg twice daily, depending on the severity of infection.
Elderly
No dose adjustment needed unless severe renal impairment.
Renal_impairment
CrCl 21-60 mL/min: 75% of standard dose. CrCl <20 mL/min: 50% of standard dose.
How to Take
Can be taken with or without food. For oral suspension, shake well before use. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to lysis and death of bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations reached in 2-6 hours.
Excretion
Mainly renal (approximately 50% unchanged in urine), and some biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect the extent.
Metabolism
Not extensively metabolized. Primarily excreted unchanged.

- Hypersensitivity to cefixime or other cephalosporins
- History of anaphylactic reactions to penicillin
Drug Interactions
Warfarin
Increased prothrombin time (INR) with concurrent use, requiring INR monitoring.
Probenecid
Increases cefixime plasma levels and prolongs half-life.
Carbamazepine
Increased carbamazepine levels.
Oral Contraceptives
May decrease efficacy of oral contraceptives (use alternative contraception).
Storage
Store at room temperature (below 30°C), protect from light and moisture. Keep reconstituted suspension refrigerated (2-8°C).
