Overview of the medicine
Zunami 500 mg Tablet contains Ciprofloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat a wide range of bacterial infections including urinary tract infections, respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted infections.
Uses & Indications
Dosage
Adults
Typically 250-750 mg orally twice daily, depending on the type and severity of infection.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function; however, dose may need to be adjusted based on creatinine clearance.
Renal_impairment
Dose reduction is necessary for patients with severe renal impairment (creatinine clearance < 30 mL/min). For example, 250-500 mg every 18-24 hours.
How to Take
Zunami 500 mg Tablet should be swallowed whole with a glass of water, with or without food. Avoid taking it with dairy products or calcium-fortified juices. It is important to complete the full course of treatment as prescribed by the doctor.
Mechanism of Action
Ciprofloxacin acts by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Within 1-2 hours of oral administration.
Excretion
Primarily excreted renally (approximately 50-70% as unchanged drug and metabolites), with some excretion in feces (15-30%).
Half life
Approximately 3-5 hours (can be prolonged in renal impairment).
Absorption
Rapidly and well absorbed from the gastrointestinal tract (bioavailability 70-80%). Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Partially metabolized in the liver to four active metabolites (desethyleneciprofloxacin, sulfociprofloxacin, oxociprofloxacin, and formylciprofloxacin), but these are less potent than the parent compound.
Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- •Co-administration with Tizanidine due to increased risk of adverse effects.
- •History of tendinitis or tendon rupture associated with quinolone use.
Drug Interactions
Warfarin
Enhanced anticoagulant effects, requiring INR monitoring.
Tizanidine
Increased risk of hypotension and sedation.
Theophylline
Increased theophylline levels and potential toxicity.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
Antacids (containing aluminum, magnesium, calcium), Iron, Zinc supplements
Reduced ciprofloxacin absorption. Take ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In the event of an acute overdose, gastric emptying by induced emesis or gastric lavage may be performed. The patient should be carefully observed and given supportive treatment. Adequate hydration should be maintained. Hemodialysis or peritoneal dialysis is not effective in removing ciprofloxacin from the body.
Pregnancy & Lactation
Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
