Description
Overview of the medicine
Zybex-SR 150 mg Tablet is an antidepressant used to treat major depressive disorder and as an aid to stop smoking. It is a sustained-release formulation of bupropion hydrochloride.
Uses & Indications
Dosage
Adults
For depression, typically initiated at 150 mg once daily. May be increased to 300 mg once daily after 3 days. For smoking cessation, 150 mg once daily for 3 days, then 150 mg twice daily.
Elderly
Consider lower starting doses and slower titration due to potential decreased renal/hepatic function.
Renal_impairment
Reduced dosage and/or frequency may be necessary; consult prescribing information.
How to Take
Take the tablet whole, do not crush, cut, or chew. Can be taken with or without food. Maintain consistent dosing times.
Mechanism of Action
Bupropion is a norepinephrine-dopamine reuptake inhibitor (NDRI). It selectively inhibits the neuronal reuptake of dopamine and norepinephrine, resulting in increased extracellular concentrations of these neurotransmitters in the brain.
Pharmacokinetics
Onset
Antidepressant effects may take several weeks (e.g., 2-4 weeks) to become apparent. Smoking cessation effects may be seen within 1-2 weeks.
Excretion
Primarily excreted renally (approximately 87% via urine) and fecally (approximately 10%).
Half life
Bupropion's half-life is approximately 14 hours; active metabolites have longer half-lives (20-37 hours).
Absorption
Well absorbed orally, peak plasma concentrations typically occur 3-6 hours post-dose for SR formulations.
Metabolism
Extensively metabolized in the liver, primarily by CYP2B6, to active hydroxylated metabolites (hydroxybupropion, threohydrobupropion, erythrohydrobupropion).
Side Effects
Contraindications
- Seizure disorder or history of seizures
- Anorexia nervosa or bulimia nervosa (current or past diagnosis)
- Abrupt discontinuation of alcohol, benzodiazepines, barbiturates, or antiepileptic drugs
- Concomitant use with MAOIs (within 14 days of use)
- Hypersensitivity to bupropion or any component of the formulation
Drug Interactions
MAOIs
Increased risk of hypertensive crisis and seizures. Contraindicated.
Alcohol
May increase the risk of neuropsychiatric events (e.g., seizures).
Levodopa/Amantadine
May increase the risk of CNS toxicity (e.g., restlessness, agitation).
SSRIs/TCAs/Antipsychotics
Bupropion is a CYP2D6 inhibitor, potentially increasing plasma levels of drugs metabolized by CYP2D6.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include seizures, hallucinations, loss of consciousness, tachycardia, and arrhythmias. Management is supportive, including activated charcoal, airway management, and control of seizures. Hospitalization is usually required.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Bupropion and its metabolites are excreted in breast milk; advise caution/consider alternatives during breastfeeding.
