Description
Overview of the medicine
Zypred (prednisolone acetate ophthalmic suspension) is a corticosteroid medication used to treat inflammation of the eye (e.g., uveitis, allergic conjunctivitis, keratitis).
Uses & Indications
Dosage
Adults
Instill 1 or 2 drops into the conjunctival sac 2 to 4 times daily. During the initial 24 to 48 hours, the dosage may be safely increased to 2 drops every hour. Care should be taken not to discontinue therapy prematurely.
Elderly
No specific dosage adjustment required.
Renal_impairment
Not applicable for topical ophthalmic use as systemic absorption is minimal.
How to Take
Shake well before use. Apply drops directly into the conjunctival sac. Avoid touching the dropper tip to the eye or any other surface.
Mechanism of Action
Prednisolone acetate acts by inhibiting the inflammatory response to a variety of agents and may delay or slow healing. It stabilizes lysosomal membranes, prevents the release of destructive acid hydrolases from leukocytes; inhibits macrophage accumulation in inflamed areas; reduces adhesion of leukocytes to capillary endothelium; reduces leakage of capillaries; inhibits fibroblast formation; reduces collagen deposition; and stabilizes cellular and subcellular membranes.

Onset
Within a few hours for anti-inflammatory effects.
Excretion
Excreted mainly in the urine.
Half life
Systemic half-life of prednisolone is 2-4 hours, but topical ocular action is localized.
Absorption
Minimal systemic absorption when applied topically to the eye. Local absorption into ocular tissues.
Metabolism
Primarily metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Acute untreated purulent bacterial infections
- Fungal infections of ocular structures
- Most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella
- Mycobacterial infection of the eye
- Hypersensitivity to any component of the medication
Drug Interactions
Systemic CYP3A4 inhibitors
While systemic absorption is minimal, potent CYP3A4 inhibitors could potentially increase systemic levels if significant absorption occurs. (Generally not clinically significant for topical ophthalmic use)
Other topical ophthalmic medications
If using multiple ophthalmic medications, administer them at least 5 minutes apart.
Storage
Store upright at 15°C to 25°C (59°F to 77°F). Protect from freezing.
Overdose
Unlikely with topical ophthalmic administration. If accidentally ingested, symptomatic and supportive measures should be taken.

Pregnancy: Category C. Use only if the potential benefit justifies the potential risk to the fetus. Lactation: It is unknown whether topical ophthalmic administration of corticosteroids could result in sufficient systemic absorption to produce detectable quantities in human milk. Exercise caution.
