Description
Overview of the medicine
Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
100 mg twice daily, preferably with or after food. The maximum recommended daily dose is 200 mg.
Elderly
Generally, the same dosage as adults, but caution should be exercised and dose adjustment may be considered due to potential for renal or hepatic impairment.
Renal_impairment
Caution is advised. Lower initial doses and careful monitoring are recommended for mild to moderate renal impairment. Contraindicated in severe renal impairment.
How to Take
Take orally, swallow the tablet whole with a sufficient amount of water. It is recommended to take aceclofenac with or immediately after food to reduce gastrointestinal discomfort.
Mechanism of Action
Aceclofenac primarily acts by inhibiting the enzyme cyclooxygenase (COX), which is involved in the synthesis of prostaglandins, mediators of pain and inflammation. It has a preferential inhibitory effect on COX-2.

Within 1-2 hours for analgesic effects.
Excretion
Approximately two-thirds of the dose is excreted in the urine and one-third in the faeces as hydroxylated metabolites.

Approximately 4-4.3 hours.
Absorption
Well absorbed orally, peak plasma concentrations reached in 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to 4'-hydroxyaceclofenac and diclofenac.

Contraindications
- Hypersensitivity to aceclofenac or other NSAIDs (e.g., aspirin-induced asthma, urticaria, acute rhinitis)
- Active gastrointestinal bleeding or peptic ulcer/hemorrhage (history of recurrent ulceration)
- Severe heart failure, severe renal impairment, or severe hepatic impairment
- Third trimester of pregnancy

Lithium
Increased plasma lithium levels, leading to toxicity.

Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, and rarely diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting, and convulsions. In cases of significant poisoning, acute renal failure and liver damage are possible. Management involves symptomatic and supportive treatment, including gastric lavage or activated charcoal if ingestion was recent.
Pregnancy & Lactation
Not recommended during the third trimester of pregnancy due to potential for premature closure of the ductus arteriosus and impaired renal function in the fetus. Should be avoided during the first two trimesters unless absolutely necessary. Use with caution during lactation; small amounts may pass into breast milk.
