Description
Overview of the medicine
Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain and inflammation in various musculoskeletal and joint conditions. The 200 mg formulation is typically an extended-release or sustained-release version designed for prolonged pain relief.
Uses & Indications
Dosage
Adults
One 200 mg tablet once daily (extended-release formulation) for chronic conditions. Dosage should be individualized based on patient response and tolerability.
Elderly
Use with caution; lower effective dose should be considered. Monitor for adverse effects, especially gastrointestinal and renal.
Renal_impairment
Reduced dosage or avoidance may be necessary in severe renal impairment. Regular monitoring of renal function is recommended.
How to Take
Take orally with food or immediately after food to minimize gastrointestinal discomfort. Swallow the tablet whole with a sufficient amount of water; do not crush, chew, or break extended-release tablets.
Mechanism of Action
Aceclofenac works by inhibiting the enzyme cyclooxygenase (COX-1 and COX-2), which is responsible for the synthesis of prostaglandins, mediators of pain, inflammation, and fever. It preferentially inhibits COX-2.
Pharmacokinetics
Onset
Pain relief may be experienced within 1-2 hours; however, full anti-inflammatory effects may take longer with extended-release formulations.
Excretion
Approximately two-thirds of the dose is excreted in the urine and one-third in the feces, mainly as hydroxylated metabolites.
Half life
Approximately 4-4.3 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached approximately 1-3 hours after oral administration.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to 4'-hydroxyaceclofenac and other metabolites.
Side Effects
Contraindications
- Hypersensitivity to aceclofenac or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced hypotensive effect and increased risk of renal impairment.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, and rarely, diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, and fainting. Acute renal failure and liver damage are possible. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy due to risk of premature closure of the ductus arteriosus and renal dysfunction in the fetus. Use with caution in the first and second trimesters only if potential benefits outweigh risks. Not recommended during breastfeeding as it may pass into breast milk.

Clinical Trials
Numerous clinical trials have established the efficacy and safety of aceclofenac in various inflammatory and painful conditions, demonstrating comparable efficacy to other NSAIDs with a potentially better gastrointestinal tolerability profile. Ongoing research explores new formulations and comparative studies.
Lab Monitoring
- Complete blood count (CBC)
- Liver function tests (LFTs)
- Renal function tests (creatinine, BUN)
- Stool occult blood test (for long-term use)
Doctor Notes
- Prescribe the lowest effective dose for the shortest duration consistent with individual patient treatment goals to minimize adverse effects.
- Monitor for signs of gastrointestinal bleeding, cardiovascular events, and renal dysfunction, especially in elderly and high-risk patients (e.g., those with a history of GI disease, cardiovascular risk factors, or renal impairment).
- Counsel patients on proper administration (with food) and potential side effects, emphasizing when to seek medical attention.
- Consider gastroprotective agents (e.g., PPIs) for patients at high risk of GI complications.
Patient Guidelines
- Always take this medicine exactly as your doctor has told you.
- Swallow the tablet whole with a glass of water, do not chew or crush it.
- Take it with or after food to reduce stomach upset.
