Uses & Indications
Dosage
Adults
Dosage varies by indication. For example, for community-acquired pneumonia, 500 mg orally as a single dose on day 1, then 250 mg once daily on days 2-5. Pediatric dosage: 10 mg/kg once daily for 3 days or 10 mg/kg on day 1, then 5 mg/kg on days 2-5 (for acute otitis media).
Elderly
No dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (CrCl >10 mL/min). Exercise caution in severe impairment.
How to Take
Oral suspension should be shaken well before use. May be taken with or without food. To maximize absorption, it is sometimes recommended to take Azithromycin suspension on an empty stomach (1 hour before or 2 hours after food), especially for single high doses for specific infections like gonorrhoea or chlamydia. For typical multi-day courses, it can be taken with or without food.
Mechanism of Action
Azithromycin works by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting protein synthesis. This results in bacteriostatic effects, preventing bacterial growth.

Contraindications

Drug Interactions
Digoxin
Increased serum digoxin levels. Monitor digoxin levels.
Warfarin
Increased anticoagulant effect. Monitor INR/PT.
Nelfinavir
Increased azithromycin serum concentrations. Monitor for azithromycin side effects.
Antacids containing aluminum or magnesium
Decreased peak serum levels of azithromycin. Administer azithromycin at least 2 hours before or 4 hours after antacids.
Storage
Store the dry powder below 30°C, away from moisture and direct light. Once reconstituted, store the suspension at room temperature (20-25°C) and discard after the specified period (e.g., 10 days). Do not refrigerate unless specified by the manufacturer.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures.
Pregnancy & Lactation
