Description
Overview of the medicine
Amlodipine is a dihydropyridine calcium antagonist (calcium ion antagonist or slow channel blocker) that inhibits the transmembrane influx of calcium ions into cardiac and vascular smooth muscle. It is used to treat hypertension and angina.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, maximum 10 mg once daily.
Elderly
Initial dose 2.5 mg once daily, then titrate.
Renal_impairment
No dosage adjustment usually required.
How to Take
May be taken with or without food.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This leads to peripheral vasodilation and reduced peripheral vascular resistance, thereby lowering blood pressure and reducing cardiac workload.

Onset
Gradual; takes several hours for full effect.
Excretion
Primarily excreted in urine (approximately 60% as metabolites, 10% as unchanged drug).

30-50 hours.
Absorption
Well absorbed orally; peak plasma concentrations 6-12 hours after administration. Absolute bioavailability is 64-90%.
Metabolism
Extensively metabolized in the liver to inactive metabolites.

Contraindications
- Hypersensitivity to amlodipine or other dihydropyridine derivatives
- Severe hypotension
- Cardiogenic shock
- Significant aortic stenosis
Drug Interactions
Sildenafil
Concomitant use may lead to excessive hypotension.
Simvastatin
Limit simvastatin dose to 20 mg/day.
CYP3A4 inducers (e.g., rifampicin)
May decrease amlodipine exposure.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
May increase amlodipine exposure.
Storage
Store below 30°C in a dry place, away from light.
Overdose
Symptoms include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves symptomatic treatment and supportive measures.
