Overview of the medicine
Trimetazidine is a metabolic agent used for the long-term treatment of angina pectoris. It helps improve myocardial glucose utilization, thereby reducing ischemic symptoms without significant hemodynamic changes.
Uses & Indications
Dosage
Adults
One 30 mg modified-release tablet once daily, taken with meals.
Elderly
No specific dose adjustment is required, but caution should be exercised due to potential renal impairment.
Renal_impairment
For patients with moderate renal impairment (creatinine clearance 30-60 mL/min), the recommended dose is one 30 mg tablet every other day. Contraindicated in severe renal impairment.
How to Take
Take the tablet orally with meals. Swallow the tablet whole with a glass of water; do not chew or crush it.
Mechanism of Action
Trimetazidine selectively inhibits 3-ketoacyl-CoA thiolase, an enzyme involved in fatty acid oxidation. This shifts myocardial energy metabolism towards glucose oxidation, which requires less oxygen per ATP molecule produced, thus improving myocardial efficiency under ischemic conditions.

Gradual, typically within 2 weeks of regular administration for full therapeutic effect.

Primarily excreted in the urine, mainly as unchanged drug (about 80%).
