Description
Overview of the medicine
Linezolid is an oxazolidinone antibiotic used to treat serious infections caused by Gram-positive bacteria that are resistant to other antibiotics, including vancomycin-resistant Enterococcus faecium (VRE) and methicillin-resistant Staphylococcus aureus (MRSA).
Uses & Indications
Dosage
Adults
For uncomplicated skin and skin structure infections: 400 mg orally every 12 hours for 10 to 14 days. For more severe infections, dosage is often 600 mg every 12 hours.
Elderly
No dosage adjustment generally required based solely on age.
Renal_impairment
No dosage adjustment for mild to severe renal impairment (including hemodialysis or CAPD patients). However, the clinical significance of accumulated metabolites is unknown.
How to Take
Can be taken with or without food. Administer orally as a capsule.
Mechanism of Action
Linezolid works by inhibiting bacterial protein synthesis. It binds to the 23S ribosomal RNA of the 50S ribosomal subunit, thereby preventing the formation of a functional 70S initiation complex, which is essential for bacterial translation.

Onset
Effects seen within hours of first dose, full therapeutic effect within a few days.
Excretion
Approximately 30% excreted as parent drug in urine, 50% as metabolites in urine, and some in feces.
Half life
Approximately 4-5 hours.
Absorption
Rapidly and extensively absorbed after oral administration, with bioavailability of approximately 100%. Peak plasma concentrations achieved in 1-2 hours.
Metabolism
Primarily oxidized to two inactive carboxylic acid metabolites by a non-cytochrome P450 mediated pathway.
Side Effects
Contraindications
- •Hypersensitivity to Linezolid or any component of the formulation.
- •Patients taking monoamine oxidase inhibitors (MAOIs) or within two weeks of taking an MAOI.
Drug Interactions
MAOIs (e.g., Phenelzine, Selegiline)
Concomitant use is contraindicated due to risk of serotonin syndrome or hypertensive crisis.
Adrenergic and Serotonergic Agents (e.g., Pseudoephedrine, SSRIs)
Potential for serotonin syndrome or hypertensive reactions; use with caution.
Storage
Store at room temperature (below 30°C), protected from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Treatment should be symptomatic and supportive. Hemodialysis removes approximately 30% of Linezolid dose.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Excreted in breast milk; caution should be exercised when administered to a nursing woman.
