Description
Overview of the medicine
Airway 10 mg tablet contains Montelukast, a leukotriene receptor antagonist used for the treatment of asthma and allergic rhinitis. It helps to reduce inflammation and relax the airways.
Uses & Indications
Dosage
Adults
For asthma and allergic rhinitis: 10 mg once daily, in the evening. For EIB: 10 mg at least 2 hours before exercise, not to be repeated within 24 hours.
Elderly
No dosage adjustment is generally required. However, use with caution due to potential age-related changes in pharmacokinetics.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take orally, once daily in the evening, with or without food. For exercise-induced bronchoconstriction, take at least 2 hours before exercise and do not take an additional dose for other indications on the same day.
Mechanism of Action
Montelukast selectively blocks the cysteinyl leukotriene 1 (CysLT1) receptor, preventing the bronchoconstriction and inflammatory effects of leukotrienes. Leukotrienes are potent inflammatory mediators involved in asthma and allergic rhinitis.
Pharmacokinetics
Onset
Within 1 day for asthma symptoms. Full therapeutic effect may take several days.
Excretion
Primarily excreted in the bile and feces (approximately 86%); negligible renal excretion (less than 0.2%).
Half life
Approximately 2.7 to 5.5 hours in adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached in 2-4 hours for adults.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (CYP3A4, CYP2C9, and CYP2A6) in the liver.
Side Effects
Contraindications
- Hypersensitivity to montelukast or any component of the formulation.
- Acute asthma attacks (not indicated for reversal of bronchospasm in acute asthma).
Drug Interactions
Rifampin
May decrease montelukast plasma concentrations by approximately 40%, but no dose adjustment is routinely recommended.
Phenobarbital
May decrease montelukast plasma concentrations by approximately 40%.

Overdose
Symptoms of overdose include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive; no specific antidote is known. Consult a healthcare provider immediately.

Pregnancy Category B. Use in pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Montelukast is excreted in breast milk; use with caution in breastfeeding mothers.
