Description
Overview of the medicine
Alatrol 10 mg Tablet contains Loratadine, which is an antihistamine used to relieve symptoms of seasonal and perennial allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps in reducing sneezing, runny nose, itchy or watery eyes, and itching.
Uses & Indications
Dosage
Adults
10 mg once daily.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal and hepatic function. For patients with impaired renal or hepatic function, refer to relevant sections.
Renal_impairment
For adult and pediatric patients (6 years and older) with liver failure or renal insufficiency (GFR < 30 mL/min), the initial dose should be 10 mg every other day.
How to Take
Alatrol 10 mg Tablet can be taken orally, with or without food, preferably at the same time each day.
Mechanism of Action
Loratadine acts as a selective peripheral H1-receptor antagonist. It blocks the action of histamine, a natural substance in the body that causes allergic symptoms, by preventing it from binding to H1-receptors, thereby reducing allergic reactions.
Pharmacokinetics
Onset
Within 1 to 3 hours after administration.
Excretion
Approximately 40% eliminated in the urine and 42% in the feces over a 10-day period, primarily as conjugated metabolites.
Half life
Loratadine: 8.4 hours (range 3 to 20 hours). Desloratadine (active metabolite): 27.8 hours (range 8.8 to 92 hours).
Absorption
Rapidly and well absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are reached in 1-1.5 hours for loratadine and 1.5-3.7 hours for desloratadine (active metabolite).
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 3A4 (CYP3A4) and CYP2D6, to its major active metabolite, desloratadine.
Side Effects
Contraindications
- Hypersensitivity to Loratadine or any component of the formulation.
- Severe hepatic impairment (caution recommended, dose adjustment may be necessary).
Drug Interactions
Cimetidine
Can increase plasma concentrations of loratadine and desloratadine.
Erythromycin
May increase plasma concentrations of loratadine and desloratadine, with potential for increased adverse effects.
Ketoconazole
Concomitant use may increase plasma concentrations of loratadine and desloratadine, leading to increased exposure and potential for adverse effects.
Other CYP3A4 or CYP2D6 Inhibitors
Co-administration with inhibitors of CYP3A4 or CYP2D6 should be done with caution as it may lead to increased loratadine levels.

Store in a cool and dry place, below 25°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, tachycardia, and headache. In case of overdose, symptomatic and supportive treatment should be initiated. Patients should be closely monitored. Gastric lavage may be considered if appropriate.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Loratadine and its active metabolite are excreted into breast milk. Therefore, use is not recommended in nursing mothers unless the potential benefits outweigh the potential risks.
