Overview of the medicine
Metronidazole suspension is an antibiotic and antiprotozoal medication used to treat a wide range of bacterial and parasitic infections. It belongs to the nitroimidazole class of drugs.
Uses & Indications
Dosage
Adults
Amoebiasis: 400-800 mg three times daily for 5-10 days. Anaerobic infections: 400-500 mg every 8 hours. Dosage varies by indication and severity.
Elderly
Generally, no specific dosage adjustment is required unless there is significant hepatic impairment. Monitor for adverse effects.
Renal_impairment
In severe renal impairment (CrCl <10 mL/min), dose reduction (e.g., 50%) or extended dosing interval may be considered, but generally not required for mild to moderate impairment.
How to Take
Take orally, preferably with or after food to minimize gastrointestinal upset. Shake the suspension well before each use. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Metronidazole is a prodrug. After entering the cell, it is reduced by susceptible anaerobic bacteria and protozoa, forming reactive free radicals that damage DNA and other vital biomolecules, leading to cell death.

Side Effects
Contraindications
- •Known hypersensitivity to metronidazole or any other nitroimidazole derivatives.

Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, abdominal cramps, flushing, headache) when consumed during treatment and for at least 3 days after stopping metronidazole.
Lithium
May increase plasma lithium levels, leading to lithium toxicity. Monitor lithium levels.
Cimetidine
May prolong metronidazole half-life by inhibiting its metabolism, increasing plasma levels.
Phenytoin, Phenobarbital
May decrease metronidazole plasma levels by increasing its metabolism.
Warfarin and other oral anticoagulants
Potentiates anticoagulant effect, increasing risk of bleeding. Requires close INR monitoring and possible dose adjustment of anticoagulant.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children. Discard any unused portion after the specified period (e.g., 14 days after opening).
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, and mild disorientation. Management is primarily supportive; gastric lavage may be helpful if ingestion is recent. No specific antidote is available.
Pregnancy & Lactation
Pregnancy Category B. While animal studies do not indicate harm, metronidazole should be avoided during the first trimester of pregnancy if possible. Use during later trimesters and lactation only if clearly needed and the benefits outweigh the risks. Metronidazole is excreted into breast milk; consider temporary discontinuation of breastfeeding.
