Alcot 800 mg Tablet contains Piracetam, a nootropic agent that works by protecting the brain and nervous system against hypoxia and improving cognitive function, memory, and learning. It is used in conditions like cortical myoclonus, vertigo, and cognitive impairment.
Uses & Indications
Dosage
Adults
Cortical myoclonus: Initially 7.2 g/day in 2-3 divided doses, increased by 4.8 g/day every 3-4 days to a max of 24 g/day. Vertigo: 2.4-4.8 g/day in 2-3 divided doses. Cognitive impairment: 2.4 g/day in 2-3 divided doses.
Elderly
Same as adults, but dose adjustment may be necessary in case of renal impairment.
Renal_impairment
Dosage must be adjusted based on creatinine clearance (e.g., CrCl 50-79 mL/min: 2/3 of usual dose; CrCl 30-49 mL/min: 1/3 of usual dose; CrCl < 20 mL/min: contraindicated).
How to Take
Oral. Tablets should be swallowed whole with water, with or without food.
Mechanism of Action
Piracetam is believed to influence neuronal and vascular functions and affect cognitive function without acting as a sedative or psychostimulant. Its exact mechanism is not fully understood, but it is thought to improve the function of acetylcholine neurotransmitter via muscarinic cholinergic (ACh) receptors, which are involved in memory processes. It may also enhance membrane fluidity and neuroplasticity.

Onset
Varies depending on the condition, generally within hours to days for noticeable effects.
Excretion
Excreted unchanged primarily via renal excretion. Over 90% eliminated in urine within 30 hours.
Half life
Approximately 4-5 hours in plasma; 6-8 hours in cerebrospinal fluid.
Absorption
Rapidly and completely absorbed after oral administration. Peak plasma concentrations are reached within approximately 1-1.5 hours.
Metabolism
Not metabolized in the human body.
Side Effects
Contraindications
- •Hypersensitivity to Piracetam or other pyrrolidone derivatives
- •Severe renal impairment (creatinine clearance < 20 mL/min)
- •Cerebral hemorrhage
- •Huntington's chorea
Drug Interactions
Warfarin
Concurrent use with Piracetam has been reported to prolong prothrombin time (INR) in some patients.
CNS depressants
May have additive effects, but generally Piracetam does not act as a CNS depressant.
Thyroid hormones
Confusion, irritability, and sleep disturbances have been reported with concomitant use of Piracetam and thyroid extract (T3 + T4).
Anticoagulants/Antiplatelets
Potentiation of antiplatelet effect, caution is advised.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include gastrointestinal upset or agitation. Treatment is symptomatic and supportive. There is no specific antidote. Hemodialysis can remove Piracetam.
Pregnancy & Lactation
Pregnancy: Not recommended during pregnancy unless clearly necessary, as there is insufficient data on human pregnancy. Animal studies have not shown teratogenic effects. Lactation: Piracetam is excreted in breast milk; therefore, breastfeeding should be avoided or the drug discontinued.
