Overview of the medicine
Amifen 500 mg Tablet contains Mefenamic Acid, a non-steroidal anti-inflammatory drug (NSAID) used for the relief of mild to moderate pain, inflammation, and fever. It is particularly effective in treating menstrual pain and other musculoskeletal pains.
Uses & Indications
Dosage
Adults
Initial dose 500 mg orally, followed by 250 mg every 6 hours as needed. Not to exceed 7 days of treatment.
Elderly
Use with caution. Start with the lowest effective dose and monitor renal function due to increased risk of adverse effects.
Renal_impairment
Not recommended in patients with severe renal impairment. In mild to moderate impairment, dose reduction may be necessary with close monitoring.
How to Take
Take Amifen 500 mg Tablet orally, preferably with food or milk to minimize gastrointestinal upset. Do not crush or chew the tablet.
Mechanism of Action
Mefenamic Acid works by inhibiting the production of prostaglandins, which are chemicals in the body that cause pain, inflammation, and fever. It achieves this by reversibly inhibiting cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes.

Onset
Analgesic effect usually starts within 1-2 hours.
Excretion
Approximately 52-67% is excreted in the urine and 20-25% in the feces, mainly as metabolites.
Half life
The plasma elimination half-life is approximately 2-4 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are usually achieved in 2-4 hours.
Metabolism
Primarily metabolized in the liver by the cytochrome P450 enzyme CYP2C9 to 3-hydroxymethyl mefenamic acid and 3-carboxy mefenamic acid.
Side Effects
Contraindications
- •Hypersensitivity to mefenamic acid, aspirin, or other NSAIDs.
- •Patients with active peptic ulceration or chronic inflammatory bowel disease.
- •Patients with a history of aspirin-sensitive asthma, urticaria, or other allergic reactions to NSAIDs.
- •Severe renal or hepatic impairment.
- •Last trimester of pregnancy (due to risk of premature closure of fetal ductus arteriosus).
Drug Interactions
Lithium
May increase plasma lithium levels.
Methotrexate
May increase methotrexate toxicity.
Diuretics (e.g., Furosemide)
May reduce the natriuretic effect of diuretics.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Other NSAIDs and Corticosteroids
Increased risk of gastrointestinal adverse effects.
Antiplatelet agents (e.g., Aspirin), SSRIs
Increased risk of gastrointestinal bleeding.
ACE Inhibitors and Angiotensin Receptor Blockers (ARBs)
May diminish the antihypertensive effect and increase the risk of renal impairment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and occasionally diarrhea. More severe cases may lead to acute renal failure, coma, or convulsions. Management involves symptomatic and supportive treatment. Gastric lavage, activated charcoal, and ensuring adequate hydration may be helpful. Urine alkalinization may enhance excretion.
Pregnancy & Lactation
Pregnancy Category C (first two trimesters), D (third trimester). Should be avoided in the third trimester due to potential adverse effects on the fetal cardiovascular system. Avoid in nursing mothers as Mefenamic Acid is excreted in breast milk.
