Amlosartan 10 mg tablet, containing Amlodipine, is a calcium channel blocker used to treat high blood pressure (hypertension) and certain types of chest pain (angina). It works by relaxing blood vessels, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, maximum 10 mg once daily. Dosage should be adjusted according to individual patient response and tolerance.
Elderly
No specific dose adjustment is typically required, but a lower starting dose (e.g., 2.5 mg) may be considered, and careful monitoring is advised.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment. Amlodipine is not dialyzable.
How to Take
Take orally once daily with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Amlodipine selectively blocks the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. It exerts its antihypertensive effect by directly relaxing vascular smooth muscle, leading to a reduction in peripheral vascular resistance and blood pressure. For angina, it reduces myocardial oxygen demand by dilating peripheral arterioles and coronary arteries.

Contraindications

Drug Interactions
Simvastatin
Coadministration of Amlodipine with Simvastatin should be limited to 20 mg Simvastatin daily due to increased risk of myopathy.
Clarithromycin
Increased risk of hypotension and acute kidney injury with concomitant use. Monitor blood pressure and kidney function.
Grapefruit juice
May increase Amlodipine exposure, leading to increased hypotensive effects.
CYP3A4 Inducers (e.g., Rifampicin, Carbamazepine, Phenytoin)
May decrease plasma concentrations of Amlodipine, reducing its efficacy.
CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir)
May increase plasma concentrations of Amlodipine, leading to increased hypotensive effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves symptomatic treatment, including cardiovascular support, elevation of extremities, and administration of IV fluids. Vasopressors may be used if needed. Calcium gluconate may be beneficial to reverse the effects of calcium channel blockade.
Pregnancy & Lactation
