Amlosartan 5 mg Tablet is a medication used to treat high blood pressure (hypertension) and chest pain (angina). It belongs to a class of drugs known as calcium channel blockers. It works by relaxing blood vessels, allowing blood to flow more easily, which lowers blood pressure and reduces the workload on the heart.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily. Max 10 mg once daily. Dose adjustment should occur after 7-14 days.
Elderly
Initial dose 2.5 mg once daily, may be increased cautiously. Use with caution due to decreased clearance.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
May be taken with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Amlosartan inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. It is a dihydropyridine calcium antagonist (calcium ion antagonist or slow channel blocker) that inhibits the transmembrane influx of calcium ions into cardiac and vascular smooth muscle. The contractile process of cardiac muscle and vascular smooth muscle depends upon the movement of extracellular calcium ions into these cells through specific ion channels. Amlosartan inhibits calcium ion influx across cell membranes selectively, with a greater effect on vascular smooth muscle cells than on cardiac muscle cells. This relaxation of vascular smooth muscle reduces peripheral vascular resistance, leading to a reduction in blood pressure.

Onset
Gradual onset, typically 6-9 hours for maximal effect on blood pressure.
Excretion
Approximately 60% of the administered dose is excreted in the urine as inactive metabolites; 10% as unchanged drug, and 20-25% via feces.
Half life
30-50 hours (terminal elimination half-life)
Absorption
Well absorbed after oral administration, bioavailability is 64-90%. Peak plasma concentrations occur 6-12 hours after administration.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Amlosartan or other dihydropyridines

Drug Interactions
Simvastatin
Increased exposure to simvastatin; limit simvastatin dose to 20 mg/day.
Clarithromycin
Increased exposure to amlosartan, monitor for symptoms of hypotension.
Dantrolene (IV)
Risk of hyperkalemia in case of severe interaction.
CYP3A4 Inducers (e.g., rifampicin, St. John's Wort)
Decreased plasma concentrations of amlosartan, potentially reducing efficacy.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
Increased plasma concentrations of amlosartan, potentially leading to increased hypotensive effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly a reflex tachycardia. Management involves symptomatic treatment and supportive measures. Clinically significant hypotension may require active cardiovascular support, including elevation of extremities, fluid administration, and vasopressors if appropriate. Calcium gluconate may be beneficial to reverse the effects of calcium channel blockade.
Pregnancy & Lactation
Pregnancy Category C. Amlosartan should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether amlosartan is excreted in human milk. Caution should be exercised when amlosartan is administered to a nursing woman.
