Overview of the medicine
Amotid-CL 500 mg Tablet is a combination antibiotic containing Amoxicillin and Clavulanic Acid. Amoxicillin is a penicillin-type antibiotic that fights bacteria, while Clavulanic Acid prevents bacteria from destroying Amoxicillin, thereby allowing it to work effectively. It is used to treat a wide range of bacterial infections.

Dosage
Adults
The usual adult dose is one Amotid-CL 500 mg tablet (500 mg Amoxicillin / 125 mg Clavulanic Acid) every 8 hours, or one 875 mg/125 mg tablet every 12 hours, for 7-10 days, depending on the severity of infection.
Elderly
No specific dose adjustment is required unless there is severe renal impairment.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance <30 ml/min. For severe impairment (CrCl <10 ml/min), the recommended dose is 500 mg/125 mg every 24 hours.
How to Take
Take the tablet whole with a glass of water, preferably at the start of a meal to minimize potential gastrointestinal intolerance.
Mechanism of Action
Amoxicillin works by inhibiting bacterial cell wall synthesis, leading to bacterial lysis and death. Clavulanic Acid is a beta-lactamase inhibitor that binds irreversibly to and inactivates beta-lactamase enzymes produced by certain bacteria, which would otherwise degrade Amoxicillin and render it ineffective. This combination extends the antibacterial spectrum of Amoxicillin.

Antibacterial effect typically begins within 1 hour of administration.
Excretion
Both components are primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Small amounts are excreted in feces.
Half life
Amoxicillin: approximately 1-1.3 hours; Clavulanic Acid: approximately 1 hour.
Absorption
Both Amoxicillin and Clavulanic Acid are well absorbed after oral administration, with peak plasma concentrations occurring approximately 1-2 hours after dosing. Absorption is optimized when taken at the start of a meal.
Metabolism
Amoxicillin is partially metabolized; Clavulanic Acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Amoxicillin, Clavulanic Acid, or any other penicillin antibiotic
- •History of cholestatic jaundice or hepatic dysfunction associated with Amoxicillin/Clavulanic Acid treatment
- •History of severe allergic reaction to another beta-lactam agent (e.g., cephalosporin, carbapenem)
Drug Interactions
Probenecid
Decreases renal tubular secretion of Amoxicillin, leading to increased and prolonged blood levels of Amoxicillin.
Allopurinol
Increased incidence of skin rash when co-administered.
Methotrexate
May increase methotrexate toxicity due to reduced renal excretion.
Oral Anticoagulants (e.g., Warfarin)
Increased prothrombin time (INR) may occur. Close monitoring of coagulation parameters is recommended.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include gastrointestinal upset and fluid and electrolyte imbalance. Treatment should be symptomatic and supportive, including gastric lavage or emesis. Hemodialysis may be useful to remove the drug from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Both Amoxicillin and Clavulanic Acid are excreted into breast milk; caution should be exercised when administering to a nursing mother.
