Description
Overview of the medicine
Ampexin 250 mg Injection contains ampicillin, a broad-spectrum penicillin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Typically 250 mg to 1 g intramuscularly or intravenously every 4 to 6 hours, depending on the severity of infection.
Elderly
Dosage adjustment may be necessary in elderly patients with impaired renal function. Monitor renal function.
Renal_impairment
Dosage should be reduced in patients with significant renal impairment. Creatinine clearance should guide dosage.
How to Take
Administer by deep intramuscular injection or by slow intravenous injection over 3-5 minutes, or by intravenous infusion. Reconstitute powder with sterile water for injection.
Mechanism of Action
Ampicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Rapid, typically within minutes after IV administration, slightly longer for IM.
Excretion
Primarily excreted unchanged in the urine via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and completely absorbed after intramuscular injection. Peak plasma concentrations are achieved within 30-60 minutes.
Metabolism
Minimal hepatic metabolism; a small fraction is metabolized to penicilloic acid.
Side Effects
Contraindications
- Hypersensitivity to penicillin antibiotics
- Infectious mononucleosis
- History of cholestatic jaundice or hepatic dysfunction associated with ampicillin
Drug Interactions
Probenecid
Increases ampicillin levels by decreasing renal tubular secretion.
Allopurinol
Increases the risk of skin rash.
Tetracyclines
May antagonize the bactericidal effect of ampicillin.
Oral Contraceptives
May reduce efficacy of oral contraceptives.
Storage
Store below 30°C, protected from light and moisture. Reconstituted solution should be used immediately or within a specified time if refrigerated.
Overdose
Symptoms of overdose include gastrointestinal disturbances (nausea, vomiting, diarrhea) and hypersensitivity reactions. In severe cases, neurological effects (e.g., seizures) may occur. Treatment is supportive; hemodialysis can remove ampicillin from the blood.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy, but use only if clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential side effects (e.g., diarrhea, candidiasis).

Doctor Notes
- Always check for penicillin allergy before administration.
- Monitor renal function, especially in elderly or renally impaired patients.
- Counsel patients on completing the full course to prevent resistance.
Patient Guidelines
- Complete the full course of antibiotic as prescribed, even if symptoms improve.
- Do not share this medicine with others.
- Inform your doctor if you develop a rash or severe diarrhea.
