Description
Overview of the medicine
Amucef 200 mg Capsule contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is effective against various Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
Most infections: 250 mg twice daily for 7-10 days. For more severe infections or certain UTIs: 500 mg twice daily. For pharyngitis/tonsillitis: 250 mg twice daily.
Elderly
No specific dosage adjustment required based on age, but renal function should be considered.
Renal_impairment
Creatinine clearance 10-29 mL/min: 250 mg or 500 mg once daily. Creatinine clearance <10 mL/min: 250 mg or 500 mg every 48 hours.
How to Take
Oral administration. Should be taken with food to enhance absorption and reduce gastrointestinal discomfort.
Mechanism of Action
Cefuroxime Axetil works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, thereby preventing the cross-linking of peptidoglycan units, which are essential for the structural integrity of the cell wall. This leads to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations achieved within 2-3 hours after oral administration.
Excretion
Primarily excreted unchanged by the kidneys, through glomerular filtration and tubular secretion.
Half life
Approximately 1.2 to 1.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and rapidly hydrolyzed by esterases to cefuroxime. Oral bioavailability is approximately 37-52% when taken with food.
Metabolism
Not significantly metabolized in the liver; largely excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to cefuroxime, other cephalosporins, or any component of the formulation.
- •History of severe hypersensitivity reaction to any type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
Drug Interactions
Antacids
Drugs that reduce gastric acidity may result in a lower bioavailability of cefuroxime axetil. Concurrent administration should be avoided.
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve by 50%.
Oral Contraceptives
Cefuroxime may decrease the efficacy of oral contraceptives. Advise women to use an alternative or additional method of contraception.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms include cerebral irritation leading to convulsions. Management includes supportive care and symptomatic treatment. Hemodialysis can reduce serum cefuroxime levels.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Cefuroxime is excreted in human milk in low concentrations; caution should be exercised when administered to a nursing woman.
