Description
Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat a variety of bacterial infections. It is effective against both Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg twice daily. For uncomplicated gonorrhea, a single 400 mg dose.
Elderly
No specific dose adjustment generally needed unless severe renal impairment.
Renal_impairment
For CrCl 21-60 mL/min, reduce dose to 300 mg daily. For CrCl <20 mL/min, reduce dose to 200 mg daily.
How to Take
Can be taken with or without food. To minimize gastric upset, it can be taken with food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Onset
Within 1 hour (for therapeutic levels).
Excretion
Primarily excreted unchanged in urine and bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect the extent.
Metabolism
Not extensively metabolized; about 10% is metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or other cephalosporins.

Drug Interactions
Warfarin
May increase anticoagulant effect (INR monitoring recommended).
Probenecid
May increase cefixime levels by decreasing renal excretion.
Carbamazepine
May increase carbamazepine plasma levels when co-administered.
Storage
Store in a cool, dry place below 30°C, away from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. No specific antidote; treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk in small amounts; use with caution in nursing mothers.
